A RESOLUTION ACCEPTING AND AUTHORIZING THE EXECUTION
OF A GRANT AGREEMENT BETWEEN THE VILLAGE OF MOUNT PROSPECT
AND THE ENVIRONMENTAL PROTECTION AGENCY.
PROSPECT MEADOWS SANITARY DISTRICT GRANT
WHEREAS, The State of Illinois enacted an Environmental Protec-
tion Act which provides in part for the control of water pollu-
tion in Illinois; and
WHEREAS, the Pollution Control Board pursuánt to said Act has
adopted regulations establishing water quality standards and
effluent limitations which must be attained by the Village of
Mount Prospect in the operation of its sewage treatment plant
and associated works; and
WHEREAS, grant funds have been provided by the State of Illinois
to assist local governments in the construction of facilities
which will ensure compliance with the standards and limitations
established by the Environmental Protection Act and regulations
adopted pursuant thereto; and
WHEREAS, the Village of Mount Prospect has made application for
a grant from the State of Illinois for the design and construc-
tion of a sewage treatment improvement and/or modification
project which will satisfy the requirements of the Environmental
Protection Act and the regulations adopted pursuant thereto; and
WHEREAS, the Environmental Protection Agency of the State of
Illinois has made an offer of grant for said project designated
as C17-2985, a copy of which is attached as Exhibit A hereto;
and
WHEREAS, the corporate authorities, after due investigation and
consideration, have determined that acceptance of said grant
offer and entering into a grant agreement with the State of
Illinois will serve the public good and is necessary to ensure
compliance with the Environmental Protection Act and regulations
adopted thereunder;
NOW, THEREFORE, BE IT RESOLVED by the Mayor and the Board of
Trustees of the village of Mount Prospect, Cook County, Illinois:
SECTION ONE: That the Mayor and Board of Trustees hereby find
as facts all of the recitals in the Preamble of this Resolution
as well as the description of the project contained in the grant
offer (Exhibit A hereto).
SECTION TWO: That the Mayor and Board of Trustees hereby
accept said offer and agree to all terms and conditions thereof,
including all special conditions, general conditions and assur-
ances contained therein and made a part hereof by reference and
also agree that the grant funds awarded will be used solely for
the purposes of the project as approved by the Environmental
Protection Agency of the State of Illinois in accordance with
the terms and conditions of .said grant offer.
SECTION THREE: That the Village Manager is authorized and
directed to execute the said grant offer on or before September
5,1989, a copy of which is attached hereto as Exhibit A.
SECTION FOUR: That the clerk be and is hereby directed to
file a certified copy of this Resolution with the said executed
grant offer and transmit sufficient copies of the same to the
Environmental Protection Agency of the State of Illinois at
Springfield, Illinois on or before September 15,1989.

effect from and after its passage and approval as provided by
law.

Infiltration/Inflow (I/I)within the former boundaries of the now dissolved Prospect
Meadows Sanitary District. Specifically, the project consists of the following:

Dear Mr. Dixon:
It is a pleasure to advise that your Step 3 wastewater treatment works grant
application for Build Illinois funds to be administered pursuant to the
procedures and rules of the Anti-Pollution Bond Act, has been approved in the
amount of $523,599. Please be advised that the Grant Amount cannot exceed 70%
of the estimated eligible project cost including the design allowance.
The grant is 70 percent of the estimated costs for the eligible project scope,
all as defined in the Grant Agreement enclosed in triplicate.
After the Agreement is reviewed, the original and one copy should be signed by
'the appropriate official and returned to the Grant Administration Section,
Division of Water Pollution Control, by September 15,1989. The second copy
is for your records. Receipt of a written refusal or failure to return the
properly executed documents within the specified time will subject the Grant
Agreement to cancellation consideration.
Your attention is invited to the Special Conditions of the Grant Agreement
including the grant p~ment schedule. Your compliance with the grant
conditions will facilitate our processing and your receipt of grant payments.
The construction contract and Change Order 1 for sanitary sewer relining work
with Insituform Midwest is eligible in the amount totaling $434,573.
The construction contract and Change Order 1 for chemical grouting work with
Visu Sewer Clean and Seal is eligible in the amount totaling $16,620.
The consulting engineering agreement for Step 3 is approved in accordance with
the requirements as contained in the State General Conditions. Receipt of a
written refusal to comply with any applicable access to records requirements
in accordance with the State General Conditions or failure to return the
properly executed documents within the specified time will be cause for.
termination of this grant offer.

The Agency has made arrangements to pay for the production and shipping of the

any resources at its disposal for installation of the sign. The sign for your
project will be ordered at this time. It will be shipped to the Mount
Prospeçt Public Works Department, 1700 West Central Road within 45-60 days.
Any inquiries regarding delivery of your sign should be addressed to Marla
Gregory of our Administrative Support Unit at 217/782-2027.
Also enclosed with this letter is a sample Resolution of Authorization. This
resolution must be completed by your governing body and returned with the
accepted grant. The grant must be signed by the person authorized by the
resolution.
For your convenience, we are forwarding a copy of this letter and the Grant
Agreement to your consulting engineer.
Your cooperation in promptly submitting the items identified in the attachment
to this letter on a timely basis will not only facilitate our administrative
handling of this project, but most importantly~ will ensure your prompt
receipt of grant payments.
