AMBULANCE RE-CHASSIS BID RECOMMENDATION
The Fire Department developed specifications and received bids to re-chassis
Ambulance 520. This vehicle is scheduled for replacement in the 2002 vehicle
replacement schedule. Ambulance 520 is a 1995 Med-Tec ambulance that has 68,265
miles.
GENERAL BID INFORMATION
On July 9, 2002, at 10:00 AM, sealed bids were received for the re-chassis of a 1995
ambulance. There were a total of eleven bids mailed' to ambulance manufacturers. A
total of five bids and one no bid were received at the time of the bid opening. The
sealed bids were publicly opened and read aloud.

RE-CHASSIS POLICY
The Fire Department has established a practice of re-chassising ambulances twice
before purchasing new. This practice is anticipated for this ambulance and will extend
the useful life of the ambulance module for a period of eighteen years. This practice
reduces the long-term cost of the vehicle. The cost of a new ambulance of this type is
currently estimated at over $100,000.
RECOMMENDATION
Based on the above information, I recommend the contract be awarded to Osage
Ambulance in Linn, Missouri, in an amount not to exceed $62,839.00 for the re-chassis
of Ambulance 520. Osage Ambulance has been manufacturing ambulances for 20-
years and have good references. Also, as required in the bid specification, Osage
Ambulance will provide a loaner ambulance during the estimated 90-day completion
period.
BUDGET AUTHORIZATION
Funding for the Ambulance re-chassis is provided in the 2002 Budget-in the Capital
Improvement Section on page 294. The account number is 6707709-660120. The
budgeted amount is $85,000.

REQUEST FOR PROPOSAL APPROVAL FOR STATION
12 TEMPORARY LIVING-QUARTERS

Prior to the start of the remodeling of Fire Station 12, which is scheduled to begin
around September 16, 2002, accommodations need to be made for temporary
living quarters for the on-duty firefighters due to the nature and scope of the
project. This is typically done using a rental trailer or mobile office. Planning for
these accommodations began in April of this year.
We contacted the Village of Arlington Heights, which is in the process of
remodeling a fire station to obtain information on what they are using for temporary
living quarters. Mike Reynolds, the Superintendent of Maintenance, for the
Arlington Heights Public Works Department advised us that they are using a rental
trailer for their temporary living quarters and provided us with a list of three vendors
to contact that can provide this type of trailer.
Having secUred three potential vendors, we proceeded to determine the space
need requirements for the trailer. We determined the trailer would need to be
large enough to accommodate up to five firefighters per day, have a sleeping area,
kitchen with eating area, day room, and a washroom with showers and lockers.
Once the needs were determined, a concept floor plan was developed (plan
attached) and a request for proposal (RFP) was sent to the three vendors for
pricing. The proposed trailer is twenty-four feet wide by fifty-feet long.
These trailers are custom made to accommodate the needs of the particular
customer. Because this is a lease contract, the vendor retains ownership and
maintenance responsibility for the unit throughout the lease period.

Recommendation
It is recommended that the Village Board award the contract to Pac-Van Leasing &
Sales, in Elmhurst, IL for an amount not to exceed $14,390.00.
Funds are available in account number 5307701-640003 Station 12 Improvements,
which is on page 289 of the 2002 budget book.
