SUBJECT: AMBULANCE PURCHASE — NORTHWEST MUNICIPAL
CONFERENCE BID
The Fire Department is requesting Village Board approval for the purchase of a
new 2012 Ford F450 Medtec ambulance from Foster Coach Sales, Inc. in an
amount not to exceed $176,311. Foster Coach Sales was awarded the
ambulance contract through the Northwest Municipal Conference competitive bid
process.
Background
The vehicle being replaced is vehicle 521, which is a 2006 Ford ambulance with
62,490 miles. The modular box on this vehicle is 12 years old and was
remounted once in 2006 as part of our remount program. Based on the vehicle's
current condition, however Public Works is recommending we not try to remount
this box on a new chassis a second time. Therefore, a new ambulance is
recommended.
The new ambulance will have a lifetime warranty, including parts and labor, on
the electrical system and body, The chassis will have the standard three year,
36,000 mile warranty from Ford.
In addition to the style being recommended, our ambulance committee also
evaluated the other ambulance type available through the Northwest Municipal
Conference Purchasing Program. This vehicle is a larger medium duty style
ambulance priced at $217,000. It was the committee's consensus that the lower
priced F450 is better suited to our needs. The F450 included in the conference
bid was $200,708, but after modifying the specifications to meet our needs, the
quote for our vehicle was considerably less.
Currently, there is $160,000 budgeted for this replacement. In addition, we have
negotiated a sale price of $22,500 for vehicle 521 to a private ambulance
company in Arkansas, which previously purchased one of our ambulances.

Vehicle 521 will not be released until the new ambulance is delivered, which will
be approximately 180 days after receipt of the chassis. A trade -in quote from the
new ambulance manufacturer would have netted the Village only $3,500. The
budgeted amount of $160,000 plus the sale price of $22,500 for vehicle 521
totals $182,500, which provides sufficient funds for this purchase.
Vendor Information
Foster Coach Sales, Inc. is located in Sterling, Illinois and has been in business
since 1957. They offer a complete service facility with certified mechanics, loaner
vehicles, remount capabilities, body shop, and an extensive parts inventory. We
have purchased ambulances from them in the past with exceptional quality and
service.
Funding and Pre-Payment Discount
The sale of vehicle 521 for $22,500 along with the funds in account number
061.80.86.00.1.000.660.005 Fire Department Vehicles, which is located on page
279 of the 2012 budget book, are available for this purchase.
The actual cost of the new ambulance is $181,946. Foster Coach Sales,
however, is offering the Village a discount of $5,635 if we prepay the full amount
at the time of sale. We consulted the Finance Director, Dave Erb, regarding this
option and he supports this pre- payment option, bringing the final total to
$176,311.

SUBJECT: WELLNESS SERVICES CONTRACT
The Village has had a wellness program for the past five years. The progr is
designed around voluntary participation of employees in a semi - annual vobllness
assessment in which the employee could benefit from reduced monthly health
insurance premiums through regular participation. The existing program has had limited
success in number of participants and was designed preliminarily as a first step until a
more robust system with a better feedback loop became available.
Village staff working with our insurance broker (GBS) has proposed a wellness product
in which employees would have a blood draw with nurse follow up and interaction with a
medical professional if the blood draw revealed a condition worthy of additional
attention. The Village currently offers an annual blood draw through the Human
Services Department, but that draw is more passive in nature in which participants are
expected to follow up on their own with a medical professional based on their results.
However, under this new program employees will be contacted directly if a condition
warrants additional follow up. Furthermore, the vendor, CHC, will be able to confirm
employee actions addressing any condition found to be necessary of follow up. The
Village staff will not be provided any medical information related to an employee and will
only receive confirmation of employee involvement in a program.
The proposed plan is to continue to leverage wellness participation through financial
incentives of reduced monthly premiums for employees. The projected maximum cost of
the program is $125 per participant for not more than 395 participants (includes retirees)
or $49,375. The benefits of the program will allow employees to have an active
snapshoot of their health through an annual wellness screening with this initial blood
draw serving as a baseline. There are other diagnostic tests included in the screening
which will complement any annual physical that is already provided at no cost to the
employees. The projected annual cost for the health insurance program for 2011 -12 is
$5.2 million so this additional cost is very minimal when compared to the overall cost of
the entire program.

There are sufficient funds available in the health insurance reserve account to cover this
program and it will be evaluated after one year. I would request Village Board
authorization to enter into a contract for diagnostic screening and on site blood draw
with CHC Wellness, Inc. in an amount not to exceed $49,375 for a period of one year
with an option to renew subsequent years thereafter.

SUBJECT: THERMAL IMAGING CAMERA REPLACEMENT
Request
The Fire Department is requesting Village Board approval to purchase four replacement
thermal imaging cameras.
Background
Thermal imaging cameras have become one of the most important tools in the fire
service in the last fifteen years. The ability to see through heavy, dense smoke to
locate fire and victims has been invaluable to us. Thermal imaging cameras provide a
heat -based contrast between items that emit heat and their immediate surroundings.
This contrast is instrumental during conditions with little visibility in finding victims and
locating the seat of a fire.
The four proposed cameras will replace existing cameras on our front -line engines and
tower ladder. In addition to these cameras, the department has four other units on the
three front line ambulances and the reserve engine for a total of eight cameras.
All of our current cameras are Mine Safety Appliances (MSA) brand, one of the highest
performing cameras in the industry. The unanimous recommendation of our committee
members, for the purposes of consistency and quality, is to stay with MSA. MSA has
been the brand of choice since our first camera was donated to us in 1998 by Wal -Mart.
The reliability and durability of the brand has been remarkable with little to no repairs
required beyond routine maintenance. In addition, consistency of this tool across all
units is paramount in maximizing its operation as multiple branded cameras could
create confusion and reduce efficiency.
At this time, there are no thermal imaging cameras on state bid. Air One Equipment
from South Elgin has provided a quote of $31,180 for the purchase of four cameras and
is the sole supplier of MSA cameras in the Chicago metropolitan area (see attached).

Two other quotes were received for thermal imaging cameras. ESG Safety Equipment,
which distributes Bullard brand cameras, exceeded MSA's prices on all of its cameras
with a quote of $41,200 on its low end camera. Municipal Emergency Services, which
distributes Scott brand cameras, provided a quote of $24,664 for its high -end camera. It
should be noted that the Scott cameras being quoted lack some notable durability
features which are included in the MSA camera. These features include a fully -
enclosed battery compartment (requirement under revised NFPA standard), dual
handles, dust and water protection, flame- resistant materials, and a larger sensor size.
Recommendation
Based on the information provided and the proven track record of the MSA cameras, I
recommend the purchase of four MSA Evolution 5200HD2 Thermal Imaging cameras
including batteries, universal truck chargers, and retractable lanyards from Air One
Equipment in an amount not to exceed $31,180.
Budget Authorization
The replacement of these cameras was approved as part of the 2012 Capital
Improvement Budget. Funds are available for this purchase in account number
020.70.90.00.1.709.665.043, which can be found on page 274 of the budget book.
