Background
State Representative Carolyn Krause acquired a State of Illinois Department of Aging grant
for $200,000 for the Village of Mount Prospect. The Human Services Department along with
the Police and Fire Departments have split the funds between them in an effort to benefit as
many Senior Citizens needs as possible. Items purchased through this grant must be
ordered before June 30, 2008, the end of the State's current budget cycle. Because of this,
staff is requesting a bid waver on the items described in this memo.
Discussion
The Police Department plans to purchase thirty-three (33) Zoll AED (automatic external
defibrillator) Plus machines for a quoted price of $47,448.95. Twenty- seven (27) machines
will be installed in the squad cars for first responders and six (6) machines will be placed in
key locations throughout the Village Hall and Public Works buildings.
The Fire Department currently owns four (4) Philips Medical Systems Cardiac
Monitor/Defibrillators. These units were purchased in 2002 and are scheduled to be
replaced in 2010, but due to the Grant, we have funds available to purchase the monitors
immediately. The changing technology and the anticipated increased maintenance cost
warrant the replacement of these units for a cost not to exceed $70,689.51
The Human Services Department plans to purchase multiple items that will enhance staff's
ability to provide services to Senior Citizen residents including Emergency Kits, LCD
Readers, Medical Lending Equipment, Home Delivered Meal supplies, a portable copy
machine to assist AARP volunteers during Income Tax season and 10 Knox Boxes for
emergency access of mobility impaired resident's homes for a total not to exceed
$81,801.05.
Recommendations
I recommend that the formal bid process be waived due to the time constraint of the Illinois
Department of Aging grant and the items listed in this memo be purchased for the amount
not to exceed $199,939.51. See attachments for more detail

Discussion
In an effort to benefit as many Senior Citizens as possible, staff recommends purchasing
the following items. These items will provide assistance to a wide range of resident's
needs from low vision to support during emergencies.
In line with our emergency preparedness efforts we are looking to better equip our at-risk
residents. The elderly and disabled are especially vulnerable during these events, and
are more than likely unequipped with supplies. The American Red Cross has provided a
quote for the purchase of 798 Emergency Preparedness Kits at a cost of $59.57 each for
a total cost of $51,059.80.
Over the years, staff have fielded many requests from residents with low vision on how
to obtain a Video Magnifier machine. These expensive machines expand pictures and
reading materials to a very large magnification so those with conditions such as Macular
Deterioration of the Retina may once again see items relatively clear. The department
will loan the machines to residents on an as needed basis. The lightHouse for the Blind
and Visually Impaired provided a quote for the purchase of five (5) Merlin LCD Readers
at a cost of $2,195 each for a total of $11,350.
A number of popular items in the Medical Leading Closet have been unavailable when
requested by infirmed residents or family members. Mark Drug provided a quote for the
purchase of eight (8) Companion Wheelchairs at a cost of $300.00 each for a total of
$2,400.00, eight (8) Wheelchairs at a cost of $600.00 each for a total of $4,800, four (4)
Shower Chairs at a cost of $60.00 each for a total of $240, and miscellaneous items
including reachers, canes, shoes horns, etc. for a total of $505.00 for a total of $7,945.

Dynamic Living provided a quote for the purchase of ten (10) SuperPole Basic (bed aid)
at a cost of $149.99 and ten (10) Advantage Rail (bathroom aid) at a cost of $189.99
each for a total of $3,449.80.
The following items will enhance the level of service provided by staff:
Apex provided a quote for the purchase of 250 once a day Pill Boxes, 100 twice a day
Pill Boxes, and 100 four times a day Pill Boxes for a total of $1,200.00.
Moore Medical provided a quote for the purchase of Blood Pressure and Cholesterol
Machine supplies for a cost of $722.16.
Support Plus provided a quote for the purchase of twenty-six (26) Telemergency Alert
Devices, which will call a pre-programmed phone number during times of emergency to
be loaned to residents on an as need basis at a cost of $119.95 each for a total of
$3,135.65.
The WEBstraurant Store provided a quote for the purchase of twelve (12) Insulated
Food Carrier Bags for the Home Delivered Meal Program at a cost of $27.99 each for a
total of $355.61.
Frys.com provided a quote for the purchase of a portable copy machine to replace the
currently broken machine for the AARP Income Tax Volunteers at a cost of $410.99 and
Office Depot provided a quote for the purchase of the copy machine cart at a cost of
$256.04 for a total of $678.03.
The Mount Prospect Fire Department loans Knox boxes to residents with severe medical
problems. Many of the program participants include senior citizens with terminal illness
or seniors recovering from surgery with impaired mobility. The participants of the
program may not be capable of opening their entry door for fire department personnel
during an emergency. Knox boxes are secured key boxes which contain keys to the
residence. The boxes are mounted permanently near the door or only temporarily with a
door hanger. These boxes provide fire department personnel rapid entry in the home
during fire or medical emergencies. Presently, the fire department only has three boxes
to loan residents on a temporary basis. The purchase of ten additional residential Knox
boxes with door hangers will allow more residents the opportunity to take advantage of
this program. The cost to purchase ten residential Knox boxes with door hangers is
$1,905.
Recommendation
I recommend that the formal bid process be waived due to the time constraint of the
grant and that the items listed in this memo be purchased for a not to exceed amount of
$81,801.05.

ISSUE
In 2007, the police department purchased four (4) Zoll AED Plus automatic external
defibrillators (AED) for use in the department. The automatic external defibrillators are located
near the department's work out facility, in the jail, in the main lobby of the Public Safety
Building, and near the Investigations Section. The department trained all personnel in the use of
cardiopulmonary resuscitation and automatic external defibrillators as part of first aid training
conducted in 2007. During the training, the department's personnel became familiar with the
operation of the Zoll AED Plus machine during practical exercises.
In May 2008, the Illinois Department of Aging awarded the Mount Prospect Human Services
Division a $200,000 grant with a portion of the funds earmarked for the acquisition of automatic
external defibrillators for the police department's patrol vehicles. As a result of this funding
secured by Illinois State Representative Carolyn Krause, staffs intention is to equip twenty-
seven (27) police vehicles with automatic external defibrillators, allowing for immediate
response to a medical emergency by police personnel until the fire department's paramedics
arrive at the scene. In addition, six (6) automatic external defibrillators would be placed in key
locations at the Mount Prospect Village Hall. Staff s recommendation is to purchase thirty-three
(33) automatic external defibrillators and related accessories through this grant with costs not to
exceed $47,448.95.
DISCUSSION
The Zoll AED Plus machines are simple, user-friendly devices to operate and their compact
space allows for easy storage. The devices are powered by standard batteries available through
retail establishments. Based on the use of the Zoll AED Plus at the Public Safety Building and
the Mount Prospect Village Hall Community Center, the training already received by police
department personnel, and a desire to maintain uniformity in the equipment deployed, staff
recommends the purchase of the Zoll AED Plus to fulfill the purchase obligations of the Illinois
Department of Aging grant.
Twenty-seven (27) police patrol vehicles would be equipped with the Zoll AED Plus automatic
external defibrillators, CPR First Responder Kits, Pedi Padz II defibrillators pads for children
and infants, and carry cases. Six (6) Zoll AED Plus machines would be mounted in designated
locations in the Village Hall using wall mount cabinets. Each of the six (6) wall mounted

cabinets would include a wall sign identifying the location of the AED and a CPR First
Responder Kit.
Staff is requesting the waiver of three (3) vendor bids for this purchase as the funds are provided
through a grant which does not require a bid process for purchases. SOS Technologies in
Chicago provided a quote for the purchase of thirty-three (33) Zoll AED Plus machines at a cost
of $1,325.00 each, thirty-three (33) Pedi Padz II at $75.00 each, and six (6) wall mount cabinets
at $239.00 each. Included in the total quoted price of $47,448.95 are thirty-three (33) police
carry cases, thirty-three (33) CPR First Responder Kits, and six (6) wall signs at no charge and a
shipping charge of $39.95. A copy of the SOS Technology quote is attached to this
memorandum.
CONCLUSION AND RECOMMENDATION
In conclusion, staff requests the formal bid process be waived to purchase thirty-three (33) Zoll
AED Plus machines and accessories as listed above from SOS Technologies of Chicago, Illinois
to fulfill a portion of the Illinois Department of Aging grant received by the Human Services
Division.

The Fire Department currently owns four (4) cardiac monitor/defibrillators. These units
were purchased in 2002 and are scheduled to be replaced in 2010, but due to a recent
Grant received from State Representative Carolyn Krause, we have funds available to
purchase the monitors immediately. The changing technology and the anticipated
increased maintenance cost warrant the replacement of these units. Also, the trade-in
value for the current units is $4,500 per unit.
In preparation for this purchase, Fire Department paramedics undertook an evaluation
of two (2) new-generation monitor/defibrillators. The purpose of the evaluation was to
determine which monitor/defibrillator would most appropriately meet the needs of the
community.
The Philips Heartstart monitor/defibrillator and the loll Medical monitor/defibrillator were
evaluated. The Philips and loll units are designed and engineered specifically for use
in a pre-hospital environment. These companies are the two major manufacturers of
pre-hospital defibrillator/monitors and were the only two considered in our evaluation.
The Philips monitor/defibrillator had significant advantages over the loll. I recommend
the Philips unit for the following reasons:
. Dual battery system for redundant backup
. Q-CPR Technology which monitors compression rate/depth
. Location of all monitoring connections makes it easier to connect cables
for external devices such as blood pressure monitoring, Sp02 monitoring,
Microstream Capnography, and Q-CPR Measurement and Feedback
modules
. Better warranty - 2 year bench warranty with loaner
. Larger display

Philips Medical has offered to extend its warranty period to two years. Zoll offered only
the standard one year warranty. Philips offer presents the opportunity to realize a
saving in operating expenses over the next couple of years.

Recommendation
It is recommended that due to the features of the Philips Monitor/Defibrillator as well as
the recent Grant received by the Village, the Village Board waive the bid process and
authorize the purchase of four (4) Philips monitor/defibrillators (see attachment) from
Philips Medical Systems out of Seattle, Washington in an amount not to exceed
$70,689.51.
Funds are available from the State of Illinois Department On Aging Grant which was
received by the Human Services Department.

BACKGROUND
As part of our annual preventive street maintenance program we typically select streets that have
been paved in the last 4-5 years for crack sealing. At 4-5 years, the pavement develops
transverse and longitudinal cracks, which are sealed to prevent further deterioration of the
pavement, thereby extending the life of the street. In order to contain the costs of such services
and material, we enter into a joint contract through the Northwest Municipal Conference
(NWMC) for this program.
BID RESULTS
Bids were received and evaluated by the NWMC for the proposed 2008 Crack Sealing Contract.
Last year, the Village paid $.987 per pound for crack sealing material installed which included a
.02 per pound NWMC administrative fee. This year the lowest responsible bidder was Complete
Asphalt Service at $.987 per pound plus a .02 MWMC administrative fee; for a total of $1.007
per pound or an increase of $.02 per pound.

DISCUSSION
Complete Asphalt Service is an IDOT approved contractor and background checks were
performed by the NWMC as this is the first time award for this company. The NWMC review
panel awarded the contract to Complete Asphalt Service as the low successful bidder for the
2008 Crack Sealing contract.
Funds for this proposed contract are; on page 206 in the current budget, account code 0505104-
540515 in the amount of $52,448 and on page 209, account code 0015108-540515 in the amount
of $5,902. The total budget amount for the 2008 program is $58,350. Our quantities of crack
sealing will be monitored so that the total expenditure does not exceed the budgeted amount.
RECOMMENDATION
I recommend acceptance of the lowest NWMC bid as submitted by Complete Asphalt Service for
an amount not to exceed $58,350.

provided are from the current Illinois state contract.

BACKGROUND
The Village owns the property behind 1818 Bonita Avenue where Well House buildings 8 & 9
are located. Currently these buildings are being used for storage for special event decorations and
seasonal equipment. The yard is fenced in with a 6' high cyclone fence topped with barbed wire
and has solid metal slats to act as a screen. The fence was originally installed 33 years ago and
has been repaired several times. Currently the fence is in severe need of repair with several bent
or damaged poles, topped with barbed wire which is a code violation and some metal slats are
missing. We have received complaints from neighboring residents that the fence does not
compliment the aesthetics of adjacent backyards.
We are recommending replacing the metal cyclone fence with a 6' high, board on board cedar
fence with metal posts. This style of fence has been used in several other Village projects and is
less institutional looking in appearance. The board on board type fence would provide screening
for the local residents and allow the wind to pass through the fence minimizing any possible
damage. The cedar boards would be allowed to weather naturally and eliminate the need for
painting or staining.
DISCUSSION
Sealed bids were publicly opened at 10:00 A.M. on June 2, 2008 for a proposed 6' high board on
board cedar fence replacement and removal and disposal of the old fence. Bid prices were based
on estimated footage and quantities which will be adjusted to the actual measured installation.
Five invitational bids were mailed, and a bid notice was placed in the local paper.

A bid from Meier's Outdoor World was received late and therefore disqualified and returned to
Meiers.
DISCUSSION
The successful low bidder, Cardinal Fence, Inc. has previously worked for the Village on
numerous projects and has performed satisfactorily. Since Cardinal's bid of $20, 763 is based on
estimated quantities, I am recommending the award be increased to $25,000 to allow for any
unforeseen obstacles found during the removal and replacement of the fence and final measured
quantities.

SUBJECT: REQUEST TO PURCHASE EQUIPMENT FOR WIRELESS AC
POINTS
Northwest Central Dispatch System (NWCDS) is implementing a new mobile data communications
system, which includes new mobile client software for Police and Fire in-car laptops. Wireless
access will be needed to install the new mobile clients and to keep them updated. To meet this
need, staff is requesting approval to purchase the equipment needed for the wireless access points.
These access points are included in the Village's technology plan and will eventually be used by
multiple departments for their daily operations.
Backqround
After researching various wireless solutions, Information Technology staff members have
determined that a Cisco Unified Wireless Network solution would best fit the Village's needs. This
solution will integrate well into the Village network and is scaleable to meet future needs. All traffic
from the access points will be directed through one of two WLAN (wireless local area network)
controllers. Each of the controllers will act as a failover for the other in the case the first is
inaccessible. A WCS (Wireless Control System) server will be used to track, monitor, and manage
the controllers and access points, and to create analysis reports.
The Cisco Unified Wireless Network solution provides WPA2 (Wi-Fi Protected Access 2) security,
which is currently the highest level of security on the market. Authentication and authorization can
be integrated with the Village's network authentication, providing an additional layer of security.
The solution utilizes 802.11 n Draft 2 technology, which is expected to be ratified in June 2009, so it

The benefits of the 802.11 n include increased speed (six times the theoretical speed of 802.11 g)
and increased range (twice the theoretical distance of 802.11 g), while retaining compatibility with
existing 802.11 big clients. Additionally we can expect the new 802.11 n technology to double the
throughput speed of the existing 802.11 big clients because of the use of MIMO (multiple-input and
multiple-output) antennas.
Currently the Police Department has five standalone wireless Proxim access points which they use
to upload videos from squad cars, and one standalone Cisco access point used for testing. Cisco
has offered a minimal amount for the old equipment for trade-in, which should be considered since
the equipment is not directly compatible with the Unified solution and does not offer the advantages
of centralized management, increased security, and increased speed and range.
Costs
Since the Village is in the process of purchasing Cisco telephone equipment, Cisco has offered
additional price breaks if we order the wireless equipment through our phone vendor at the same
time the phone equipment is ordered. With this additional discount from Cisco, Sound Incorporated,
our phone vendor has quoted a 42% discount off list price. RMS Technology Solutions, our
networking consultant, has quoted a 36% discount. CDW prices run 31% to 34% below list price.
Sound Incorporated's quote is attached. We expect to order the phone equipment the week of June
16th, so we would need to order the wireless equipment at the same time to qualify for the 42%
discount.
The quote includes thirteen access points. Five will replace the existing Proxim access points at the
Police Department and two will be placed at the main fire station. Also two are designed for Public
Works, one for each of the outlying fire stations, and one for Village Hall (potentially for guest
access in the Board Room). The other one will be used as a spare since it is not cost effective to
purchase maintenance support on these units. Additional access points will most likely be added at
a later date.
Configuration of the equipment, which will be performed by RMS Technology Solutions, is estimated
at $1,665. The initial site at the main fire station should not require a site survey, but some of the
other sites may require a survey.
Recommendation
I recommend that the Village purchase the equipment for the wireless access points described in
Attachment A of this document from Sound Incorporated for an amount not to exceed $21,481.
Funds are available for these purchases. Please feel free to contact me with any questions or

The Village is considering building improvements at three locations. Topographic surveys are
necessary at all three sites to provide the information needed to complete the design of the
building improvement projects. The three locations are:
o FIRE STATION 14/2000 EAST KENSINGTON ROAD
o PUBLIC WORKS FACILITY / 1700 WEST CENTRAL ROAD
DWELL # 16: / 1629 IMPERIAL COURT
The project design will require the collection of field data including elevations, above ground
features and underground utilities and the preparation of topographic survey drawings of the
three sites.
Proposal Results
Seven firms were solicited for proposals and five firms returned proposals on June 6, 2008. The
proposals supplied references of past work and qualifications of the personnel who would be
involved in the design. Engineering firms were asked to submit as part of their proposal the
following information:
1. A summary ofthe firms approach to the project.
2. A staffhour estimate for major phases of work.
3. A maximum, not-to-exceed fee for the project.
4. A project schedule.

Discussion
All the firms that responded to the RFP have the experience and expertise to complete the work.
All expressed an understanding of the work involved and presented approaches which would
accomplish the necessary tasks. CE Design LTD. presented a schedule which represented
adequate staff hours to complete the work and that is compatible with the desired time line for the
project. CE Design has performed other design and survey work for the Village in the past and
they have exhibited the ability to competently complete their work.
Recommendation
It is my recommendation that the proposal submitted by CE Design, LTD. for Topographic
Surveys for Fire Station 14, Public Works Facility and Well # 16 be accepted with a not to
exceed amount of$14,880.00. This expenditure has been allocated in the 2008 Village Budget.
Please include this item on the June 1 ih Village Board Meeting Agenda.

Annually the Village uses the Northwest Municipal Conference (NWMC)/Suburban
Purchasing Cooperative (SPC) bid contract to purchase the Village supply of fuel. Last
year the bid was awarded to Gas Depot Oil Company, Morton Grove, Illinois. The bid
was with three (3) additional one (1) year optional contract extensions. The
NWMC/SPC has decided not to extend the contract and they have solicited new bid
proposals.
Bid Award
On June 2, 2008 we were notified by the NWMC that this years SPC fuel bid was
awarded to Texor Petroleum Company, Riverside, Illinois. The cost of fuel per gallon
will vary according to market conditions, as reflected in the Oil Price Information Service
(OPIS) data sheets. Texor Petroleum Company's bid was for $0.0595 over the OPIS
low cost of fuel for unleaded gasoline and $0.0869 for #2 bio-diesel. The contract that
just ended with Gas Depot was for $0.060 over the OPIS low cost of both #2 bio- diesel
and unleaded gasoline. Attached is the 2007/2008 and 2008/2009 fuel bid work sheet
that reflects the last Village contract pricing versus the new SPC bid award pricing. The
bid also contained a provision to extend the contract three (3) additional one (1) year
contracts.
Discussion
The NWMC/SPC fuel bid award is in effect from June 6,2008 to June 5,2009.
Texor Petroleum Company was our supplier prior to last year's contract with Gas Depot
Oil Company. Texor Petroleum Company had performed well for nine years prior to last
year's contract with Gas Depot Oil Company.

Recommendation
It is my recommendation that the Village accept the NWMC/SPC bid award and
execute a new 2008/2009 fuel contract with Texor Petroleum Company, Riverside,
Illinois, to be in effect from June 18, 2008 to June 5, 2009 and also approve the three
one year extensions if approved by the NWMC/SPC. Funds for this proposed contract
are available in the current 2008 budget and will be in the proposed 2009 budget. (Fire
Department Vehicle Maintenance Account #0014206-570635 and Public Works Vehicle
Maintenance Account #6605702-570635).

Backaround
At the August 14, 2007 Committee of the Whole Meeting, the Village Board discussed pursuing e
development of a marketing plan. Conducting a Retail Market Analysis was identified a an
important step to help guide retail recruitment and marketing efforts. There was consensus from
the Village Board to move forward with this initiative. An RFP was issued, and Business Districts
Inc. (BDI) was retained to conduct the study.
Discussion
The purpose of the Retail Market Analysis was to determine current retail climate in Mount
Prospect, identify regional retail trends, and pinpoint the best options for moving forward relative to
our potential customer base and prepare for future retail trends. One of the primary objectives for
the project was to obtain an economic analysis that would establish a set of strategic policies to
guide the Village to the most beneficial next stage of retail development. Of particular importance is
the vitality of the downtown, the redevelopment of Randhurst Shopping Center, and the South
retail district.
Updates on the report were given to the Village Board of Trustees at the Retail 1-2-3 workshop
held on December 11, 2007 and the Branding Workshop on February 26. The preliminary report
was presented at the April 22 Committee of the Whole Meeting.
The completed report contains a situation audit; a "where-do-we-go-from-here" diagnosis of our
community's economic and physical conditions as well as market demographics and retail mix.
From this analysis, a strategic plan was developed highlighting specific, actionable steps.
Recommendation
Staff's recommendation is that the Village Board accept the final report of the Retail Market
Analysis. The report can be used to guide strategic policy decisions in terms of retail development
as well as recruitment and marketing efforts. The Retail Market Analysis will serve as a useful tool
as we move with the implementation of the Village's marketing plan.
