WHEREAS, MedTec Healthcare & Private Duty LLC ("Petitioner") has filed a petition to amend a
Conditional Use permit with respect to property located at 411 Business Center Drive, ("Property")
and legally described as follows:

WHEREAS, the Petitioner seeks an amendment to a Conditional Use permit to expand the existing
adult daycare center located at 411 Business Center Drive into adjacent suites 101 and 102 adding
three thousand, seven hundred seven square feet (3,707 SF) at a multi -tenant office building; and
WHEREAS, a Public Hearing was held on the request for a Conditional Use permit being the
subject of PZ -18-21 before the Planning and Zoning Commission of the Village of Mount Prospect
on the 111h day of November, 2021, pursuant to proper legal notice having been published in the
D,gj[y__H,erA d Nal r on the 271h day of October, 2021; and
WHEREAS, the Planning and Zoning Commission has submitted its findings and recommendations
to the Mayor and Board of Trustees in support of the request being the subject of PZ -18-21; and
WHEREAS, the Mayor and Board of Trustees of the Village of Mount Prospect have given
consideration to the request herein and have determined that the same meets the standards of the
Village and that the granting of the proposed amendment to a Conditional Use permit would be in
the best interest of the Village.
NOW, THEREFORE, BE IT ORDAINED BY THE MAYOR AND BOARD OF TRUSTEES OF THE
VILLAGE OF MOUNT PROSPECT, COOK COUNTY, ILLINOIS ACTING IN THE EXERCISE OF
THEIR HOME RULE POWERS:
SECTION ONE: The recitals set forth are incorporated as findings of fact by the Mayor and Board
of Trustees of the Village of Mount Prospect.

gj, T TiJWQ: The Mayor and Board of Trustees of the Village of Mount Prospect grant an
amendment to the Conditional Use Permit allowing an adult daycare to operate at 411 Business
Center Drive (Ordinance No. 6478) to enable expansion of the adult daycare facility as illustrated
on the preliminary Egress Plans prepared by Techno Ltd dated October 5, 202 and attached hereto
as Exhibit A and subject to the following conditions:
1) "Existing occupant load of 80 people must be maintained until expansion is completed
and may not be exceeded at any time;
2) Approval from the Illinois State Fire Marshal and Illinois Department of Aging prior to
granting expanded occupancy;
3) Compliance with all applicable Village Code requirements, including, but not limited to,
fire code and building regulations."
giECTION THRE The Village Clerk is hereby authorized and directed to record a certified copy of
this Ordinance with the Recorder of Deeds of Cook County.
