Consent
A key component of the Mount Prospect Strategic Plan is to improve the Village's
infrastructure, including identifying lighting needs within the current residential
street light system. The next segment of roadway slated for street light
improvements is Seminole Lane.
Seminole Lane is a Village collector street that straddles the dividing line between
Mount Prospect and Prospect Heights, between River Road and just west of Andoa
Lane. West of Andoa lies fully within Prospect Heights City boundaries.
In this section of road there is single family housing, multifamily housing, as well
as access to Robert Frost Elementary School, and the newly renovated Aspen
Trails Park. Also, along Seminole Lane is a recently signed and striped bike route.
All these locations generate traffic along with the daily subdivision traffic. The
neighborhood currently has ComEd owned street lights only at intersections
providing limited nighttime visibility.
Mount Prospect informed Prospect Heights of this project and invited them to
participate in continuation of the project west of Andoa into Prospect Heights to

complete the street lighting all the way to Wolf Road. Prospect Heights declined
to participate so the project limits will be from River Road to the Mount Prospect
Village limits just west of Andoa Lane.
Staff is seeking to enter into a contract with a consulting engineering firm for
design and construction engineering to facilitate construction of the lighting
improvements.
A Request for Technical and Cost Proposals was posted on the Demandstar
website. Five (5) consulting engineering firms responded and submitted Technical
and Cost Proposals. Each firm was asked to submit as part of their proposal the
following information:
1. Resumes of key personal and related experience on similar type projects
2. Summary of the firm's approach to the project
I Staff hour estimate and cost proposal for major phases of work
4. Project schedule
Tasks that are identified to be performed include:
Design Engineering
. Perform field survey
• Prepare construction plans, specifications, and cost estimates
• Coordinate design with utility companies
• Provide bidding assistance
Construction Engineering
• Provide construction staking
• Provide construction inspection and supervision
• Attend construction meetings
• Prepare pay estimates, punch -list, and as -built drawings
The consulting firms' Technical and Cost Proposals were reviewed on the basis of
their understanding of our goals and their methodology of the design process.
They were rated (with a maximum score of 100 points) according to these specific
categories: company background, experience, scope of work, schedule, work
effort and proposal content. The rankings are:

Ciorba Group has put together a very strong project team with experience in
design and construction engineering for numerous municipal street light projects.
They have a very experienced professional engineering staff with a thorough
understanding of the project coordination, communication and documentation
needs to successfully accomplish the project within the proposed schedule.
Ciorba Group successfully completed design and construction engineering for
various projects within the Village, including street lighting along Central Road.
They also completed a Village wide street lighting study and perform bridge
inspections.
It is Staff's opinion that Ciorba Group will provide the best product at an
appropriate number of hours dedicated to design and construction engineering.
Alternatives
1. Accept proposal for Seminole Lane Street Lighting Project design and
construction engineering.
2. Action at discretion of Village Board.

Duane O�Laughlim' PE - Project PrimcipaKQma0tyManaQer: Duane is [iorba'sVice President Director of Engineering'
overseeing all engineering studies, design plan preparation, and construction engineering for improvements tolocal streets,
county highways, arterial roadways, and expressways. Duane has over 36years ofexperience in engineering for public
infrastructure projects. His experience comes from I I years in the US Army assigned to the Corps of Engineers and 26 years in
the public sector both with acontractor and with consulting engineers. As Project Principal, he will be responsible for
maintaining the project schedule and bud@e1. Duane wilt also serve as the Quality Manager, verifying 1haLQC/QA procedures
have been implemented for our services.
Joseph Vomdra, PE, LC - Project Manager/Resident Engineer: Joe has over Z4years of experience designing lighting systems
for residential, collector and arterial streets; interstate expressways; interchanges; streetscaping; tunnels; underpasses; and
parking bots. These projects were designed for local municipalities, including the Village of Mount Prospect, using both local
and federal funds, the City of Chicago, |OOTand the |KinoisTo\bway. His experience includes the preparation of photometric
analysis for both HPS and LED lighting, pole layout, voltage drop calculations and controller design. Joe will supervise the
lighting design and pian preparation. During construction, Joe will oversee the work ofour Resident Technician as well as
supervise the review mfthe contractor's shop drawings to monitor conformance with the pians and specifications. Based on
examination, Joe is Lighting Certified (L[} by the National Council on Qualifications for the Lighting Professions and is a
member ofthe Illuminating Engineering Society (IES). In addition to his design experience, Joe has overseen or performed
part-time construction engineering services for several lighting and electrical projects for municipalities,
Daniel Johnson, PE- Senior Lighting Engineer: Dan has 18years ofexperience designing lighting improvements for
municipalities, |QOT' the Illinois Tc(kwayand other state transportation agencies. Dan has also recently assisted in the
development ofthe Village's Residential Street Lighting Program and completed the design of several residential street
lighting projects for, the City ofChicago. This work included photometric analysis' pole layout and voltage drop calculations
for new lighting systems. Hewilt assist Mr. Vondrain the development ofplans, specifications, and estimates for this project.
Adam Harn - Resident Technician: Adam is an Engineering Technician with an Associates degree in Engineering Technologies
and 11 years of experience in design, review of shop drawings and construction engineering services. Adam has served as a
Resident Technician /Construction Inspector on local collector street resurfacing' county highway reconstruction, water main
installation, and interstate highway rehabilitation projects. Hchas also performed construction layout services. Adam has
recently served as Resident Technician on municipal projects for Morton Grove, Rolling Meadows, Highland Park, and Hoffman
Estates. Heiucertified inDocumentation ofContract Quantities (#19'155O8)'trained in|CQRS,and attended the
Department's Erosion and Sediment Control Workshops (Fundamentals and Inspection). Adam has worked on several design
and construction projects for the Village, including the Corridor Street Lighting Evaluation Study, Meiaa Park Electrical
Upgrades, Kensington Business Park Basin Improvements, and Huntington Lift Station projects.

Mr. O'Laughlin is Clorba's Vice President- Director of Engineering and serves as a Project Manager
for preliminary, design and construction engineering services. These projects have included the

Project Manager supervising the update tothe Project Development Report and the preparation ofplans,
specifications, and cost estimates for the reconstruction of/. 1miles ofLake Street. Project limits are
from Ashland Ave »o1-90/94 (Kennedy Expmsaway.The CTA Green Line elevated tracks mnalongthe
entire length ofproject. The project will lower the street profile toincrease clearance for the CTA overhead
structure. Improvements also include the installation of new street lighting, traff ic signal modernizations,
drainage system Improvements and the removal of hazardous materials. Landscape and streetscape
enhancements will improve the corridor aesthetics. Ciorba coordinated with adjacent property owners for
the removal or replacement of existing vaulted sidewalks. Federal STP funds will be used for construction so
all plan documents tobeprepared inaccordance with |DOTrequirements.
1-90p�����Facilities, Pace.
Project Manager supervising the preliminary, design, and construction engineering for three new Park-
n-Ricle facilities along the Jane Addams Memorial TN|way(1-90). The project is part of the plan mmake
|-9Uanintermodal corridor. The three facilities are located atthe Randall Road, Barrington Road and |L25
interchanges. Each facility will include a new parking lot for a minimum of 150 passenger vehicles, separate
bus turnaround area, Kiss -n -Ride and shuttle drop-off areas, pedestrian and bicycle accommodations,
stormwater management and detention facilities, site lighting and landscaping. All construction will comply
with ADA requirements. Coordination was required with all agencies inthe project limits including the Illinois
TollwaV, the Illinois Department of Transportation, Cook County, the City of Elgin, and the Village of Hoffman
Estates.
Project Manager overseeing final design engineering for the reconstruction of80thAxenuefrnm1e1st
Street ,c1G8rdStreet, within the Villages nfTinley Park and MokemThe road will bewidened from atwo
lane rural section to a four lane urban section with auxiliary turn lanes added at crossstreets. Improvements
will be made to the intersections with 189th Street, 186th Street, 185th Street, and 183rd Street. The
existing two lane bridge carrying 80thAvenue nver|-mowiUberep|acedwithanewhour|anestmctum
Other improvements will include designing traffic signals, a multi -use path, and landscaping along the
corridor. Duane will also conduct public involvement activities to present project challenges and alternate
solutions ,ostakeholders for their review and input.
Chestnut Street, Spruce Street, and Lincoln Avenue 5treetscapeImprovements, Village ofWimnetka
Project Manager overseeing the preparation ofPhase Uplans, specifications, and estimates for suegscape
improvements on Chestnut Street and Spruce Street within the Village's Comr-nercial District. The plans were
an update to a streetscape design originally prepared by Ciorba for the entire Commercial District. RoadwaV
geometric revisions were required tomatch anew site development aLthe corner ofSpruce Street and
Green Bay Road. All intersections were designed with new ADA ramps to meet current PROWAG policy.

Joe has over 24 years of experience designing lighting systems for projects on local roads, arterial
streets, interstate expressways, and parking lots. These projects were completed for municipalities,
momnties,|DOT, and the Illinois 7o|AmayJoe oversees the photometric analysis, equipment selection
and layout, control cabinet design, voltage drop calculations, and preparation xfspecifications for new

fair traffic signal improvements associated with the reconstruction of 80th Avenue from 191st Street
to 183rd Street. The road will be widened from a two lane rural section to a four lane urban section
with auxiliary turn lanes added at cross streets. Npw traffic signals and an interconnect system
will be designed for the 80th Avenue intersections with 183rd Street and 185th Street under the
jurisdiction of Cook and Will counties, respectively. TemporarV signals required for the maintenance of
traffic will beinstalled at the 8OthAvenue intersections with 1B3rdStreet. Roadway and pedestrian
path lighting was provided for the length of the project. Other lighting work included underpass and

Project Engineer for the design of a replacement lighting system for their Village Hall Parking Lot. New LED lighting was provided for the parking lot
and adjacent streets. Provisions for future electric vehicle charging stations was included in the design. Design plans included the proposed lighting
plan, single line diagram, as well as standard and project specific details. A custom control cabinet was designed to power the lighting system and
future electric vehicle chargers. During construction, Ciorba's design staffreviewed catalog cuts submitted bythe contractor for conformance tothe

Brookfield Avenue over Salt Creek Village wfBrookfield.
Lead Lighting Engineer for final design for the replacement of the Brookfield Avenue bridge over Salt Creek. Existing Village owned light poles were
relocated to accommodate replacement of the bridge. Ciorba worked with the Village to select decorative bridge -mounted luminaires to enhance
the pedestrian path and look -out over Salt Creek. Ciorba also provided new sign and flagpole lighting at the Brookfield Village Hall.

Project Engineer for the design of two permanent cabinets supplying electricity for the Mount Prospect Lions Club Annual Village Festival that
occurs each Independence Day at Melas Park. These cabinets provide electricity for all the various food and game tents as well as some of the rides
found at the festival. An arc fault study completed by the Village revealed that the existing cabinets were unsafe and needed to be replaced prior to
the next event. Design and construction of the new cabinets was completed in five months, including the fabrication of the control cabinets, which
are typically along-lead time item.
William Street Relief Station Retrofit, Village mfMount Prospect
Electrical Engineer for the design for the replacement of pumps, controls and electric service for an existing relief station. The design included
power and communication circuits for the relief station controls, pumps, and sensors. The design allowed the existing generator and automatic
transfer switch to reduce project cost. Ciorba coordinated with ComEd to obtain a replacement service installation, as the existing service was in
poor condition.

Using the Village's CIS information, Ciorba
identified residential areas along local and
collector streets for the installation mfnew
roadway lighting. Eight alternate residential
lighting schemes ranging from fully meeting
IES nequinementstoremoval ofthe existing
residential lighting were considered. For each
alternate, standard and decorative equipment
was evaluated. Cost estimates were prepared
for each evaluation.
Recommended future action items were
identified tohelp guide the discussion of
the residential lighting program. These

the removal and replacement ofthe existing
pavement, curb and gutter, and sidewalks.
Other improvements include installation ofnew
street lighting, modernizing two existing traffic
signals, installation oftwo new traffic signals,
sewer structure replacements and repairs, and
landscaping mimprove the aesthetics ofthe
corridor. The CTA Green Line elevated track ("the
EL")runs the entire length ofthe improvement.
The pavement elevation will belowered tu
improve the vertical clearance between the
elevated track and the street. Avariable height
curb head will beused along the entire project
length mmaintain proper drainage and meet
ADA requirements. STP funding will beused for
construction; therefore, all work will bedone in
accordance with 0OTBureau ofLocal Road and
Streets (8LR&S)and FHWArequirements.
During the design ofthe project, the City noticed
frequent maintenance required for LED fixtures
attached ^oLED fixtures through the City. |twas
determined that fixtures were failing due to
excessive vibration. roavoid these issues with
new installations, [iorbaevaluated three LED
luminaire layouts: viaduct luminaires attached
tothe CTA elevated track structure, decorative
acorn style luminaires mounted l*'above
the pavement and residential poles. The City
ultimately selected the residential style poles
consisting ofan1D'mounting height davit
style pole with acobraheadstyle iuminaire
with apiggyback acorn luminaire toenhance
pedestrian lighting behind the poles. Ciorba
performed afield survey toidentify buildings
with existing vaulted sidewalks. Specialized pole
foundation details were provided where vaulted
sidewalks are mremain.

installed along Algonquin Road, Golf Road,
Irving Park Road, Meacham Road and Wise
Road, Existing HID luminaires were replaced
with LED fixtures along Meacham Road and
Wise Road. The improvement ioutilizing
Surface Transportation Program (STP)funds
for construction, therefore, the project was let
bythe Illinois Department ofTransportation
(|DOT).
Construction engineering services included
attending apeconstructionmeeting,
observing the contractor's operations for
conformance with the plans and specifications,
final quantity documentation and preparation
ofchange orders and pay estimates, The
Resident Engineer was also responsible
for coordination with utilities impacted by
construction activities and chairing weekly
progress meetings. Project documentation was
completed using | DOT's |CORSconstruction
management software since federal funds
were used for construction. OolLighting
Engineer completed areview mfcatalog cuts
submitted bythe contractor onbehalf ofthe
Village, prior tosubmittal for |oOT[iu/ba
identified and resolved several potential issues
prior tnthe contractor ordering materials,
preserving the project schedule. Record
drawings were prepared inaccordance with
Village requirements.
Joe provided construction support by
reviewing material submittals, addressing
contractor RFb,reviewing and coordinating

conflicts discovered during construction, and
supplemental field inspection onmnas-needed

The Village of Mount Prospect requires a qualified engineering firm to prepare design plans and specifications as well as
provide construction engineering services for the installation of new street Lighting system along, Seminole Lane between
the Village Limits near Andoa Lane and River Road. This project will be completed in accordance with the Village's Street
Lighting Program Guidelines and current Village standards.
Ciorba Group's Project Team will apply the skills learned from the many successful past Lighting projects completed for the
Village, state agencies and Chicago area communities, In addition to street lighting installed along Central Road, Algonquin
Road, Busse Road, Dempster Street and Main Street within the Village of Mount Prospect, Cinrbo has completed several
lighting studies for the Village. Our experience also includes the design of street Lighting projects for Tinley Park' Palatine,
Arlington Heights' Morton Grove, Rolling Meadows, Schaumburg' North Chicago, Winnetka' Illinois Department of
Transportation (1Q0T) and the KhnoisToikway. We also provide on-going Lighting design services to the City of Chicago as well
asassist in managing annual residential and arterial Lighting replacement programs. We also review designs prepared by other
consultants, review catalog cuts and prepare specifications and details as required.
Ciorbav4Uverify and implement the Village's desired 300 -foot pole spacing aseilowed bysite conditions such as the location
of existing utilities, trees and roadway geometry. Pians' specifications, and estimates will be prepared in accordance with
Village, Illuminating Engineering Society LIES) and IDOT recommendations to receive Village approval. Proposed light poles
will be Located outside of the River Road right-of-way so that an IDOT permit is not required for this project.
Construction observation services wh8 be provided in accordance with 1DQT procedures to ensure a successful project. Ciorba
Group recommends imp4ementingaoonmbinadonoffuiiandpart time construction observation services for the Lighting
installation. Fuiitime observation will be provided while underground work, such as foundation and duct installation, is being
performed. Part time observation will be provided while above ground work, such as pole and cable installation is performed.
Communication. <]orba understands that effective communication yields successful projects. [inrba^s Project
Manager will keep in close communication with the Village at at[ times. Our Project Manager and Resident
Technician will be available at any time to meet with the Village.
Attention to Detail. An important aspect of any project is attention to detail. For this project, it will be
especially true in the Location of proposed light poles toavoid conflicts with existing trees and utilities located
within the parkway.

during a walking survey in lieu of full topographic survey. Hardscape items such as pavement, curb lines, sidewalks and
driveways wilt be identified from aerial photos. Trees, vegetation' utilities and other features wit[ be Located by means of
walking survey to mitigate potential conflicts with the proposed lighting system. Aerial photos wilt be obtained from the
United States Geological Survey (U3GS) website. Utility information will be obtained bycontacting the Joint Utility Location
Information for Excavators (JULIE). Village atlases wit[ be obtained in CIS format and imported directly into CAD. [iorba*iii

calculations will be performed to verify the proper sizing of cabling between the light poles.

Pians' One Line Diagram, and electrical details. Applicable |DOT highway standards will also be included in the bid
documents. Upon completion of the preliminary design, a plan -in -hand field review wilt be performed to confirm the
proposed equipment locations. This review will include checks for potential utility conflicts and interference with parkway
trees. Village commentoand field review notes will beincorporated into final plans.
Specifications will be tailored 10the Village's requirements. Luminaire properties such as photometric distribution, color
temperature and warranty requirements will beclearly defined to ensure the Village receives the product it desires.
Quantities and cost estimates will be prepared for each submittal, preliminary and final. General Conditions, Bid
Advertisement, Instructions to Bidders, Price Schedule, Agreement, Bid Bond, and Contract Bond will also be part nfthe

Precomstruction Conference- Typical topics that may be discussed at this meeting will include the progress schedule, the
location of on-site equipment and material storage sites, utility stakeout, permits, and material inspection information.
Catalog cuts and shop drawings may be distributed at the meeting by the contractor for review by [iurba. A list ofsuppliers
and subcontractors will be furnished by the contractor for approval. JoeYondn* will serve as Resident Engineer for this
project, applying the familiarity ofthis project gained during design and his experience from previous lighting and electrical
projects designed and constructed for the Village. Joe will oversee preparation and distribution of meeting minutes for this,
and all future meetings held during construction. In addition to the preconstruction conference, Joe will chair weekly
progress meetings with the contractor, subcontractors, and the Village to discuss past and future activities, issues, and
coordination requirements. The Village and impacted utility companies, and the contractor will be invited to attend.
Catalog Cut Revhews- Oorba"s design staff wit! review the contractor supplied show drawings and catalog cuts for
conformance to the contract documents. Upon completion ufCiorba's review, comments will be forwarded to the Village for
concurrence. Lighting often involves long Lead items for delivery such as the poles. Timely reviews and approval of
submittals are essential an the contractor can order equipment and materials as soon as possible to avoid delays.
Construction Engineering and Documentation - Construction will be monitored for conformance with the contract
documents and Village requirements, Construction layout wiUbecompleted by(]orbastaff. |fthere are any unforeseen
conditions or issues that develop, [imrba's design engineers will be available to provide options to the Village to mitigate
costly delays. Ciorba will make recommendations to the Village concerning any potential pian revisions and document any
changes accordingly.
Full time construction engineering services will be provided for underground operations such as the installation of
foundations and duct. Part Lime inspection will be provided for above ground work such as pole and cable installation. Adam
Ham will serve as our Resident Technician for this project and will be on-site each day to observe aii construction activities
including traffic control, foundation installation, directional boring and pole installation. Joe will periodically visit the
project site to monitor progress, address unforeseen issues and assist Adam as needed.
Measurements and calculations will bemade daily and Inspector Daily Reports wit[ berecorded onaregular basis. Monthly pay
estimates will be submitted tothe Village for review and approval before final processing. Adam will maintain a Daily Diary
describing the work accomplished each day. Weekly progress and quantity reports will be prepared and provided tothe Village
and the contractor. All project documentation will follow IDOT and Village Standards.
Upon completion and acceptance of the new lighting system,Cimdba will coordinate with ComBto authorize removal of their
existing utility pole mounted Luminaires.
Project Close -mut- {jmrbaviU maintain a punch list ofconstruction items that require correction before final acceptance of
the project and provide it to the contractor. The site will be inspected for completion of punch list items and the contractor
wiU be notified ofany incomplete items. After the punch list is completed, awsik through will be conducted by Joe and
Adam with the Village and the contractor. Any concerns identified atthe walk through will be resolved prior, toacceptance
of the improvement. The closeout process continues with the preparation of final documentation papers for project
acceptance bythe Village. This includes all material certifications and testing results, final quantity measurements,
balancing change orders and final pay estimates. Upon final acceptance of the project, the job box will be turned over to
the Village.
Construction Record Drawings - Ciorba will update the project CAD files with any changes made to the location or routing of
equipment during construction. These changes will be field measured and documented for inclusion in the final construction
record drawings. The two sets of final construction record drawings of size specified by the Village will be provided and one
[Qwith all updated CAD files.

General Scope of Work

Project: Seminole Lane — Street Lighting Project
Location: Seminole Lane, Village limits west of Andoa Lane to River Road
The Village is interested in providing a street lighting system on Seminole Lane from the Village limits west
of Andoa Lane to River Road. Seminole Lane is a local residential street under the jurisdiction the Village
of Mount Prospect and City of Prospect Heights. The project is approximately 4400' long. Removal of the
Commonwealth Edison lights located at the intersections along the proposed route will be included with
this project. This project will provide Village street lights at approximately 300' spacing and at
intersections. New lights will not be located within the River Road ROW. A permit from Illinois
Department of Transportation will not be required as part of this project. However, coordination with the
City of Prospect Heights will be required. Design, construction, and construction engineering services will
be funded 100% by the Village.
At a minimum, the following tasks shall be included in the scope of services. The Village will consider
additional tasks at the consultant's recommendation.
Design Services:
1. Coordinate a Project Kick -Off Meeting with Village Staff.
2. Perform field survey of topography, utilities, and right-of-way features along the route of the
proposed improvement. Plans to also show Village boundaries.
3. Coordinate with public utility companies.
4. Prepare preliminary and final construction plans, details, specifications, and bid documents.
5. Prepare preliminary and final cost estimates.
6. Prepare all required permit applications and obtain approvals of plans and specifications from all
appropriate agencies as necessary, including the City of Prospect Heights.
7. Coordinate with ComEd for new service and light/pole removals.
8. Assist with the bidding process.
Construction Engineering Services:
1. Coordinate a Preconstruction Meeting with Village Staff and the Contractor.
2. Provide horizontal and vertical construction layout of the proposed improvements as necessary.
3. Provide construction supervision and inspection.
