Information
To assure that high quality asphalt and concrete materials are utilized by contractors in this year"s street resurfacing
and sidewalk improvement programs., the Public Works Department requires the services of a materials testing
consultant. To procure these services,, staff prepared and distributed a detailed request for proposals (RFP). A copy of
this document is attached. Tasks defined in the scope of work included:
1. Project Management / Report Review
2. Engineering Technician
3. Vehicle Charge
4. Cylinder pickup
5. Compression Testing per cylinder
6. Nuclear Density Gage Usage
7. Maximum Theoretical Specific Gravity
8. Bulk Specific Gravity
9. Ignition oven test / Reflux extraction and sieve analysis
RFP Results
The consultants' proposals were reviewed on the basis of their understanding of our goals and their testing
methodologies. They were rated (with a maximum score of 100 points) according to these specific categories:
Background, experience, scope of work proposed, schedule,, work effort, proposal content, and cost. The results are
denoted in the tables below:

Discussion
Applied GeoScience, Inc. has a very experienced professional staff with a thorough understanding of the material
testing requirements necessary to successfully accomplish this for the village's projects. They are prequalified by the
Illinois Department • Transportation (IDOT) to perform this work and have the capability to respond to • s•-
• • eeds. They have successfully accomplished these same services for • • in recent years
and received positive references from those communities. It is staff"s opinion that Applied GeoScience., Inc. will
provide high quality materials testing services at a reasonable cost. Applied GeoScience's proposal is attached.
The services listed in the proposal are based on estimated jobsite work hours using averages from previous years and
prorated for the anticipated work for this year. Many factors can influence the actual jobsite work hours for quality
assurance testing. In order to allow for contingencies and the possible need for additional testing,, staff recommends
that the contract award include a 20% contingency ($19,,612 + $3,923 = $23f535).

1. Accept Applied GeoSciente, Inc. proposal to provide materials testing services for 2018 Street Resurfacing and

various locations throughout Mount Prospect. The broad objectives of our work will be to perform Portland Cement
Concrete testing and Asphalt Pavement testing on an as -needed basis, as requested and directed by the owner's

Key Personnel
Adam M. Moghamis, P.E., Principal Engineer
Mr. Moghamis has over 28 years of experience in the geotechnical, environmental, and construction engineering
fields. He directs all geotechnical and materials engineering provided by the firm. He provides technical advice and
direction and reviews reports prepared by other engineers. He is involved with projects that include slope stability
analysis, embankment, vibration monitoring, subgrade stabilization, and materials testing. Mr. Moghamis has
made engineering recommendations and prepared subsurface exploration boring reports and testing programs for
a variety of commercial, industrial, and residential buildings. His involvement in soil investigations has included
triaxial testing, consolidation testing, and permeability testing as well as fills, footing, soil and rock caisson
inspections and concrete inspections. Mr. Moghamis is a Registered Professional Engineer in the states of Illinois,
Indiana, Kansas, Texas, Florida, Missouri, Ohio, and Wisconsin.
Kathleen N. Larsen, Environmental consultant
Ms. Larsen has five years of experience in the fields of civil and environmental engineering, including work in the
areas of environmental assessment and remediation, asbestos inspections, stormwater management, and
geotechnical investigations. At Applied GeoScience, Inc., Ms. Larsen manages the performance of all
environmental aspects -of projects. Ms. Larsen's Phase I experience has included assessments of commercial,
municipal, and vacant land for a variety of clients, including private developers and government agencies. She
has also performed pre -construction inspections to assess the integrity of roadways, building foundations, and
other structures prior to sewer and right-of-way work. Her Phase 11 experience has included subsurface
investigations, underground tank removal, and screenings for asbestos. She also performs field screening and
coordinates laboratory testing of soils for disposal purposes. Ms. Larsen provides cost estimates for these
operations and submits documentation and plans for site closure through state -regulated cleanup programs where
appropriate. Ms. Larsen is also responsible for the review of written reports and the coordination of field
investigation and abatement activities. She interacts with utility companies as well as local, state, and federal
agencies as required to secure appropriate permits for cleanups.
Kevin Gosiewski, Project Manager
Mr. Gosiewski has just begun working in the fields of civil and environmental engineering, performing work in the
areas of environmental assessment and remediation, construction materials testing, and geotechnical
investigations. At Applied GeoScience, Inc., Mr. Gosiewski - coordinates. field operations for the firm's
environmental, geotechnical, and construction materials sectors, as well as Quality Control/Quality Assurance
reporting and documentation. He works closely with municipalities, CDOT, I DOT, and private clientele to insure
that all materials are placed within the applicable project specifications. During his time at Applied GeoScience
thus far, Mr. Gosiewski has had experience with, project management, Phase I and Phase 11 inspections, and soil
sampling. Mr. Gosiewski has experience with managing and conducting all aspects of investigations for sites that
have enrolled in the Illinois Site Remediation Program. He has been responsible for the field investigation and
report preparation including the Site Investigation Report and Remedial Objectives Report.

Mr. Peck has just begun his experience in the fields of civil and environmental engineering, during which time he
has performed work in the areas of construction materials testing, and geotechnical investigations. At Applied
GeoScience, Inc., Mr. Peck coordinates field operations for the firm's environmental and construction materials
sectors, as well as Quality Control/Quality Assurance reporting and documentation. He works closely with
municipalities, CDQT, IDGT, and private clientele to insure that 'all materials are placed within the applicable
project specifications. He performs inspections of reinforcing steel to ensure that placement, size, spacing, and
quality are in accordance with project specifications. Mr. Peck also qualified to perform inspections of structural
steel, including bolted and welded connections, for proper placement and weld curvature. Mr. Peck's Phase
experience has included assessments of commercial, municipal, and vacant land for a variety of clients, including
private developers and government agencies. He has also performed pre -construction inspections to assess the

Material testing stipulations:
• All work shall conform to IDOT QC/QA requirements, including but not limited to:
a. IDOT Standard Specifications for Road and Bridge Construction (4/1/2016)
b. IDOT Supplemental Specifications and Recurring Special Provisions (1/1/2018)
c. IDOT Project Procedures Guide (6/1/2009)
d. IDOT Manual for Materials Inspection (4/1/2016)
e. IDOT Manual of Test Procedures for Materials (1/1/2017).
• As part of Jobsite QA, the Consultant will provide an individual to act as the QA Manager.
The QA Manager will have at a minimum a HMA Level II Technician certification.
• Services will be required from approximately April 1, 2018 to November 1, 2018.
• Field Inspection hours will begin when the Engineering Technician arrives on the jobsite;
inspection hours will end once testing is complete and the technician leaves the jobsite. The
Engineering Technician shall not exceed 8 hours on site, unless written permission is
obtained from the Village. The Consultant shall submit a field trip ticket to the Village to
verify hours on the jobsite.
• The cost for Cylinder Pickup shall include all labor, vehicles, and materials necessary to
travel to the jobsite, retrieve test cylinders, and deliver the cylinders to the lab.
• Each PCC compressive strength test gathering shall consist of casting four, 6 -inch diameter
concrete cylinders. The Consultant shall test two of the cylinders 7 days after casting, and
test the remaining two cylinders 14 days after casting. The Consultant shall submit final PCC
reports to the Village no later than 7 days after the 14 -day compressive strength test.
The Village reserves the rights to accept or reject any and all proposals, to waive any informality
in any proposals, and to accept any proposals considered advantageous to the Village.
