SUBJECT: PROPOSAL FOR AMBULANCE BILLING SERVICES
PROPOSAL
The Finance Department is requesting Village Board approval to enter into a three-year
contract for the billing and collecting of the Village's ambulance bills.
BACKGROUND
The Finance Department has been billing for non-resident ambulance service for
approximately 17 years. Currently, we send out an average of 375 bills per year.
Beginning in 2006 we will be billing residents as well as non-residents for ambulance
service and we estimate that we will be sending out 2,500-3,000 bills per year. Because of
the increased volume and the complexity of dealing with multiple insurance companies as
well as Medicare and Medicaid, it was decided to outsource the ambulance billing to a
private billing company.
DISCUSSION
Four ambulance billing companies were contacted and asked to provide information on
their services. After a review of the submittals, two companies (Medical Reimbursement
Services and Andres Medical Billing) were called in to interview based on the information
they provided and their fee structure. Representatives of the Fire Department and the
Information Technology Department assisted in the interview process.
Both firms selected for the interview have been in business for at least ten years and
provided us with a list of other municipalities for whom they are currently working. The fee
structure for both firms was based on a percentage of the amount actually collected.
Medical Reimbursement Services presented a contract based on 6% of the amount
collected. They currently have 22 client communities that are mostly in South and
Southwest suburbs. The other firm, Andres Medical Billing, serves over 120 agencies,

including many in the Northwest suburbs. Their fee was 7% of the amount collected. Both
companies certify that they are fully HIPPA compliant and both provide a toll free number
as a convenience for insurance companies and patients to call. See Attachment A for a
listing of client communities for Medical Reimbursement Services and Andres Medical
Billing.
One of the major differences between the two companies that came out during the
interviews is the way they are able to obtain ambulance billing information and the hospital
affiliations they have. The Village's Fire Department utilizes an ambulance reporting
software called GEAC that is based at Northwest Community Hospital. Most of the fire
departments in the Northwest suburbs use this software and are currently migrating to an
upgraded version. Andres Medical Billing also utilizes this software and has the ability to
be able to access the Village's ambulance reports on line as they do for other clients.
Andres appears to be better able to obtain the patient's insurance information from the
hospital. This means less direct contact with the patient. If Andres is unable to collect
insurance information from the hospital, they mail a questionnaire to the responsible party
asking for the information. The other firm, Medical Reimbursement Services, does not
utilize the GEAC software and will call patients directly if unable to obtain insurance
information from the hospital.
In order to educate residents and non-residents on the new billing procedure, a Frequently
Asked Questions (FAQ) has been prepared and will be made available through various
means. Since there will be different billing procedures depending on whether you are a
resident or non-resident, a separate F AQ has been prepared for each (Attachment B & C).
RECOMMENDATION
It is my recommendation that we enter into a three-year contract with Andres Medical
Billing to provide ambulance billing and collection services for the Village at a rate of 7% of
all ambulance billing collections. Andres has the tools necessary to obtain the required
insurance information with minimal contact with the patient and the experience necessary
to obtain payment from the various insurance providers. While Andres' fee is slightly
higher, both the Fire Department and IT felt the advantages of being on the same software
platform were significant. This will allow for a more efficient operation and less staff time
dedicated to the ambulance billing process.

· What is the cost to a Mount Prospect residentfor an ambulance service?
Cost will be based on current Medicare rates, mileage to the hospital and the level
of service provided. The resident will not directly receive a bill for ambulance
service. Your insurance company will receive a bill for the service.
· Will I receive any correspondence form the Village of Mount Prospect?
You may receive a letter from Andre's Medical Billing (the Village's billing
company) requesting your insurance information so that they can generate the
invoice. You may also receive a Customer Survey from the Mount Prospect Fire
Department
· What if my insurance company doesn't pay for the entire bill? Will I be
responsible for the remainder?
NO. The Village of Mount Prospect has instructed the billing company to only
accept what the insurance company provides. You will not be asked, nor
expected, to pay any balance that was not paid by the insurance company or
Medicare (primary and secondary coverage if applicable.)
· Will the Village of Mount Prospect bill Medicare directly?
Yes. However, it may be necessary to contact you through a letter or by phone to
obtain the information needed to be able to bill Medicare or the insurance
company
· Do I have to have the insurance information ready for the paramedics when
they respond to a call for me?
No. The main concern during an incident requiring the fire department
ambulance is your care and treatment. Insurance information will be obtained at a
later time.
· Who should I call if I have any questions regarding the care that I received or
the ambulance billing?
If you have questions regarding ambulance billing, please call Andre's Medical

Background Information on the Marked Police Patrol Squad Car Fleet
In 2000, the police department began purchasing Ford Interceptor marked patrol squad
cars as replacements for the discontinued Chevrolet Caprice squad cars. The
department completed the transition to an all Ford fleet of marked patrol squad cars the
following year. This purchase marks the beginning of the fifth rotation of the Ford
marked patrol squad car fleet.
In the time they have been in service, the Ford Interceptors have proven to be well
suited to police patrol operations. An evaluation of the Interceptor's acceleration,
braking, turning and maneuvering capabilities indicates it meets all of the department's
emergency and pursuit driving requirements. Being a large,. full frame vehicle, the
Interceptor easily accommodates the in-car computer, mobile video recorder and police
radios that are installed in the front seat area while also providing ample room to safely
and effectively transport prisoners in the back seat area. Finally, the Public Works
mechanics are satisfied with the Interceptor's durability and reliability, with the warranty
and repair services provided by Ford dealers and with Ford's responsiveness to
recommendations for vehicle improvements in subsequent model years.
Alternative Squad Car
At this time there are two (2) alternative ''full size" sedan style police pursuit vehicle
packages available, the Chevrolet Impala and the Dodge Charger. Despite being sold
as a "full size" vehicle, the Chevy Impala actually is smaller than the Interceptor and
thus does not accommodate people or equipment as well as the Ford Interceptor. The
Dodge Charger uses unibody construction and does not have a frame as the Ford

Recommendation
The department is recommending the purchase of ten (10) Ford Interceptors for use as
marked police patrol squad cars. The Ford Interceptor, in a police pursuit vehicle
package, is available from Landmark Ford in Springfield under a State of Illinois bid
award (#PSD4013240) at a cost of $21 ,500 per vehicle. As a comparison, last year the
Village purchased replacement squad cars under the state award from the same dealer
at a cost of $22,050 per vehicle. We are requesting the Village Board authorize the
purchase of ten (10) Ford Interceptor police pursuit vehicles from Landmark Ford at a
total cost not to exceed $215,000 under the pricing of the State of Illinois contract for
police vehicles. The State of Illinois contract was awarded through a competitive bid
process, and we are eligible to make purchases under the contract provisions. Funds
for eight (8) of these vehicles are available for this expenditure in vehicle replacement
account #6707709-660123, which is listed on page #292 of the 2006 Annual Budget.
Funds for two (2) of these vehicles are available for this expenditure as listed in capital
improvement account 5107701-660122 on page #291 of the 2006 Annual Budget.
These two (2) vehicles, new to the vehicle inventory, will be utilized by the department's
traffic unit. Delivery of these ten (10) vehicles will be in early 2006.

Background Information on the Unmarked Detectives Cars
The department replaces one of its fleet of nine unmarked staff (3) and detective (6)
squad cars every year, if needed. Since 1998, the department has purchased smaller,
front-wheel drive vehicles for staff and detective use. Currently seven of the
department's staff/detective cars are either Chevrolet Luminas or Chevrolet Impalas.
The remaining two vehicles are older full-size Chevrolet Caprice sedans. The
department is recommending the replacement of one of these vehicles, detective squad
1-28, a 1996 Chevrolet Caprice.
Life Cycle Cost Analysis
The staff and detective cars are on a nine-year replacement cycle. No vehicle is
recommended for replacement unless an independent assessment of the vehicle's
condition, durability, and service history by public works personnel indicates
replacement is warranted. Using these criteria, detective car 1-28, a 1996 Chevrolet
Caprice with more than 100,000 miles, has been recommended for replacement in
2006. In January 2006,1-28 will begin its eleventh year of service. An attached
spreadsheet displays the life cycle cost analysis for 1-28. The optimum economic
replacement point for 1-28 was calculated at year five, while the modified replacement
point was calculated at year ten.
Recommendation
The Chevrolet Impala sedan, in a police package, is available from Miles Chevrolet in

Decatur, Illinois under the State of Illinois bid award (#PSD4012465) at a cost of
$17,900. As a comparison, last year the Village purchased the same vehicle under the
state bid from the same dealer at a cost of $17,340. Funds are available for this
expenditure in account # 6707709-660123, which is listed on page #289 of the 2006
Annual Budget. The department is requesting the Village Board authorize the police
department to purchase one Chevrolet Impala at a cost not to exceed $17,900 under
the pricing of the State of Illinois contract for police vehicles. The State of Illinois
contract was awarded through a competitive bid process, and we are eligible to make
purchases under the contract provisions. Delivery of these vehicles will be in early

Project Shield is a Department of Homeland Security equipment grant administered by the Cook
County Board through the Cook County Bureau of Information Technology and Automation in
conjunction with the contractor, International Business Machines. The intent of the project is to
provide first responders - police, fire, and emergency services personnel - with wireless
connectivity to real time video images from police and emergency vehicles, municipally-owned
compatible camera systems, and privately-owned compatible camera systems during emergency
incidents. .
This wireless network, utilizing the existing Cook County wide area network (WAN) and
wireless wide area network (WW AN) allows incident commanders access to the cameras from
remote locations. The intent of the grant is to provide Cook County law enforcement and
emergency agencies with the technology and software necessary to start the program at no cost
to the law enforcement agencies.
The first phase of this program began with installation of equipment in May 2005. The proposal
before the village and this police department is Phase II of the Project Shield grant. Phase II will
equip fifty-six (56) additional agencies with the requisite grant equipment and software.
The Project Shield Phase II program provides the following items to the agency at no cost:
· Wireless Access Point: A site survey to determine the best location for a wireless access
point, otherwise known as a "hot spot". The Department of Homeland Security requires
the hot spot be placed near a potential terrorism target such as transportation centers,
school, large entertainment venues, etc.
· FreQuencv and Licensin2: The wireless video system will operate at both 2.4 GHz and
4.9 GHz after upgrades are completed with Phase I clients and with all Phase II clients.
The program initially operated on 2.4 GHZ, however this band-width is in the public
domain and currently is becoming overcrowded due to the proliferation of wireless access
points in residences and businesses. The higher band-width of 4.9 GHz is controlled by
the Federal Communications Commission and requires licensing. Cook County has
already obtained a 4.9 GHz experimental license. The village would need to obtain a

communications license for this band width similar to the licenses we currently possess
for the radio and data frequencies maintained through Northwest Central Dispatch.
. Wireless Access Point Location and Homeland Securitv: The wireless access point
will be equipped with a pan-tilt camera with a 176x zoom lens. The camera will be
controlled remotely from a high end network viewing station from either one of the two
(2) properly equipped vehicles or through a permanent, dedicated viewing/video
management station at a location such as the police department. All transmissions
involving these camera systems are digitally encrypted and cannot be accessed by
unauthorized users. The camera system and the method of controlling the camera are
similar to the camera boxes the Chicago Police Department is utilizing in their high crime
areas.
. Police Department Local Area Network and Workstation: The Cook County wireless
network for this equipment will be connected to the local area network (LAN) of the
police department. The grant supplies a firewall and universal power supply for the
firewall, a digital video recorder, a video server, and the hardware necessary for
establishing a viewing station at a location determined by the police department (Le.
command center, police desk, etc.). Funding provides for one (1) work station and the
municipality may be required to provide one (1) to two (2) additional work stations as
determined by the site survey.
. Video Recordine and Review: The dedicated viewing/video management station will be
equipped with a digital video recorder (DVR) and video server for the review of captured
video. The system is designed to allow the viewing of recorded material while system
continues to capture video from the camera.
. Password Maintenance and Homeland Securitv: The police department would
maintain the passwords for this system. The equipment in Mount Prospect cannot be
accessed by other law enforcement agencies unless a homeland security incident occurs.
Should a homeland security incident occur, the Cook County team incident commander
has the authority to take over the video system until the incident is deemed controlled or
terminated. Specific criteria are outlined to define a "homeland security incident."
. Police Vehicles: Two (2) police vehicles will be equipped with a Panasonic Toughbook
laptop computer, a server containing a DVD recorder and DVD burner, and the
appropriate networking and transmission hardware. All requisite software will be
provided by the contractor.
. Additional Equipment: The village has the option to purchase additional equipment
through the contract such as the necessary equipment for equipping additional police
vehicles or camera positions and wireless access points. The contractor would work with
this department in mitigating costs as the department currently possesses some of the
requisite equipment in the fleet of police vehicles.

· Shot Sootter: The contractor is to establish a pilot program for Shot Spotter. Shot
Spotter is equipment mounted on the highest location in a given area designed to 44listen"
for gunshots. Upon detection of gunshots, an alert is provided to the dispatch center and
the device will be able to map the location of the gunshots utilizing GIS within forty (40)
feet. In the testing phase of this equipment, the device must be able to map 80% of
detected gunshots within the forty (40) foot requirement. It is unknown at this time if the
Shot Spotter pilot program will be open to this department at this time.
As the program develops and grows, the village and the police department could enter into
discussions with potential targets to determine whether this system should be applied to their
facility, the conditions necessary to allow law enforcement aCCess to their video system, and
alerting such entities to federal funding available private companies and other governing bodies
to assist in the financing the equipment. Applicable locations to consider within Mount Prospect
include the petroleuni tank and pumping facilities, Randhurst Mall, schools, and rail lines.
Attached to this memorandum are copies of a Power Point presentation outlining Project Shield.
Please note some information in this Power Point is stale as the program has moved from the 2.4
GHz frequency to the dual band system of 2.4 GHz and 4.9 GHz. As a result, some equipment
changes have occurred.

COOK COUNTY PROJECT Shield
Cook County has created and is implementing a County wide First-Responder Mobile Video and
Data Network, utilizing the latest technology available. The new system will provide Cook
County, and the municipalities within Cook County with integrated, interoperable, emergency
response capabilities. For those Cook County Municipalities that want to expand their First-
Response Mobile capabilities, utilizing the Cook County solution, Cook County has made
available, through their Prime Subcontractor, IBM, the ability for the municipalities to acquire
additional equipment and services at Cook County negotiated discounted rates.
Overview Of The Cook County Solution
Cook County will provide a wireless public safety network for Cook County municipalities that
will allow police, firefighters and paramedics to share real-time audio and video, speeding
response to emergencies.
Once complete, the county's new network will enable police and emergency response agencies
within the county to access their applications and be able to send and view video from cameras
mounted within their vehicles, atop radio towers, utility poles, and inside municipal buildings.
This will allow them to more quickly coordinate efforts to ensure faster responses with the right
equipment and people. The wireless network will provide continuous connectivity throughout the
county, ensuring vehicles will have real-time access to video and data.
Additionally, the vehicles will be capable of creating their own wireless hotspots. When several
vehicles converge on the scene of a situation, they can link up to share video images allowing
first-responders to quickly assess the incident and better plan their response.
Cook County will provide video, mobile computers and communication equipment that will be
installed in first-responder vehicles and video from the vehicle cameras can be transmitted to a
cOmnland center as it happens. The first-responders will also be able to access internet and
intranet applications from mobile computer systems, providing them with increased access to
information and data while on the move.
The tower cameras will provide an eye-in-the-sky view and the strongboxes are bulletproof
camera units mounted on utility-poles in high-traffic or high-crime areas. Officials can remotely
control the tower and strongbox cameras to view incidents so they can zoom in when something
happens that requires attention.

To keep the public informed in the event of an incident, Cook County will use the Cook County
Public Safety Notification System, which allows electronic message panels located in high
citizen traffic areas, such as shopping centers, downtown areas, stadiums, municipal buildings,
and schools, to be controlled from a central location such as a Command Center.
In addition to designing the network and installing equipment, Cook County will setup and
operate a Wireless Support Center to provide help desk operations for the first - responders and
end users.

As part of the contract, all municipalities within Cook County have the ability to purchase
additional equipment at the Cook County negotiated discounted rates through IBM.

AND PURCHASE FOUR NON-INVASIVE BLOOD
PRESSURE MODULES
BACKGROUND
The Fire Department purchased four (4) portable automatic blood pressure machines in
1994, which are still in use today. These units are becoming progressively more
unreliable and due to the age of the machines, parts and service are becoming more
difficult to obtain. Therefore, we recommend replacing all of the units at the same time
for consistency purposes.
The current blood pressure monitors are stand-alone units. However, blood pressure
monitors are now available as an integrated component of our cardiac
monitor/defibrillator. Currently, loll cardiac monitors are used in all four (4) ambulances
and are capable of accepting the blood pressure monitor module.
GENERAL BID INFORMATION
In order to utilize the blood pressure monitor as an integral part of our cardiac monitor it
has to be a loll Medical Corporation product. That module is only available from loll as
a sole source. A copy of the sole source letter along with a brochure of the monitor and
non-invasive blood pressure module is attached to this memo.

BUDGET AUTHORIZATION
The cost to add the blood pressure module is $4,200 for each unit and $16,800 for all
four units. Funds are available in the Paramedic Equipment account (0014202-670014),
which can be found on page 184 of the budget book.
RECOMMENDATION
Based on the above information, I am requesting to waive the bid process and purchase
four (4) non-invasive blood pressure modules for the cardiac monitors from Zoll Medical
Corporation in Chemslford, Massachusetts in an amount not to exceed $16,800.

Per our conversation, I wanted to let 'you know lOll Medical is the only provider of NIBP
upgrades to M Series defibrillators. We do not let other vendors upgrade our equipment.
Additionally, lOLL Medical does not sell new defibrillators thru distributors or other vendors.
