The undersigned Local Governmental Entity does hereby elect, pursuant to the provisions of Sectioos 14Q4 and 14.()S
of the Illinois Un~loyment Insurance Act, to pay when due, in lieu of contributions, an amount equal to the
~t of ~gulal" benefits and the amount of exten&ed benefits paid to its 'M:>rkers or fonner ~r!ccrs for any weeks
beginning on and after the effective date of this election, OIl the basis of wages for insured 'M:>rlc paid to them
by it during the period this election is in effect. ~_
The undersigned local governmental entity makes its election with the understanding that:
1. TIle election becomes binding upon its approval by the Director of Labor) and the duration of the election
period cannot be less than one calendar year from the date the undersigned has first become subject to the
Unemployment Insurance Act) and cannot be less than two calendar yearS if the undersigned has previously in-
curred liability for the pa)'ll1ent of contributionS;
2. The undersigned local governmental entity as a self-insurer, shall be liable for reimbUr5<Ement of any S\lIll
erroneously paid as benefits on any of its 'IlOrkers or fonner ~rk:ers) unless such sum has been recovered by
the State.
3. At the close of each calendar quarter, the undersigned local governmental entity will receive a Statement of
the :>mount due fmu it for the benefits paid to its ;;orkers or former workers during the calendar quarter, and
will pay the :lmOunt due to the Di rector within 30 days after the date of mailing of the Statement. in the
event that it fails to make any payment when due, or fails to file, when due, any quarterly report of the
wages paid to its \'\Orkers, it shall be subject to the interest or penalty provisioos of the law, as the case
