AN ORDINANCE AUTHORIZING AN
ANNEXATION AGREEMENT
FOR CERTAIN PROPERTY LOCATED AT THE
SOUTHEAST CORNER OF WOLF AND KENSINGTON ROADS
(CAREMATRIX DEVELOPMENT)
WHEREAS, the Corporate Authorities of the Village of Mount Prospect, Cook
County, Illinois, did, on the 6th day of October ., 1998, hold a public hearing to
consider an Annexation Agreement for the annexation of certain property not presently
within the corporate limits of any municipality but contiguous to the Village of Mount
Prospect, said Agreement being entitled "Annexation Agreement", a true and correct
copy of which is attached hereto and made a part hereof as Exhibit "A"; and
WHEREAS, the public hearing was held pursuant to legal notice as required by
law, and all persons desiring an opportunity to be heard were given such opportunity at
the public hearing; and
WHEREAS, the Corporate Authorities of the Village of Mount Prospect, Cook
County, Illinois have determined that it is in the best interests of the Village of Mount
Prospect that the Agreement be entered into by the Village of Mount Prospect;
NOW, THEREFORE, be it ordained by the President and Board of Trustees of
the Village of Mount Prospect, Cook County, Illinois, as follows:
Section '1: The President and Board of Trustees of the Village of Mount
Prospect find that it is in the best interests of the Village of Mount Prospect and its
residents that this Annexation Agreement be entered into and executed by the Village
of Mount Prospect. The Agreement is substantially in the attached form and is made a
art of this Ordinance as Exhibit "A".
Section 2: The President and Clerk of the Village of Mount Prospect, Cook
County, illinois, are authorized to execute on behalf of the Village of Mount Prospect
the Annexation Agreement; after all of the other parties to the Agreement have properly
signed and executed the same.
Section 3: The Village Clerk is hereby directed and ordered to publish this
Ordinance in pamphlet form and to record the Annexation Agreement with the Recorder
of Deeds of Cook County, Illinois.
Section 4: This Ordinance shall take effect from and after its passage,
approval and publication in the manner provided by law.

Prosl~ V'glage Code m of the dale of this Agreement. Any ordinance, resolution, or motion of the
Village imposing any new permit tees or L~crea~ing the arnoura oft. he existing permit fees, plan
review and inspection fees, lk, ense fee not otherv~ set by this Agreement, or any other tees imposed
by the Village that are appIicable to or required to be pald by the PURCHASER, contractors,

14. CONSTRU(~rYION MA.~'AGEMENT. PLAN, The Owner 0~each lot. or its
designee, shall submit a Consm~cl~on Management Plan (the "Plin") to the V',lla~e which specifies
all m~ara of ingress and egress (including that for emergency vehicles) during con~'traztion on Lot
1 and Lot 2, as appropriate. The Plm shall also include the approximate location of all temporary

'v'~age of Mzur~ Prospect. The Traffic Regulafon Agreemcra shall include provisions for patrolling
and enforcing the ate and orderly movement, standing an~i parking ofvchicle~ on the roads, drive,
parking and loading areas. The payment ofco~s of any additional police services above and beyond
