WHEREAS, David Kim of MedTec Healthcare & Private Duty, LLC ("Petitioner") has filed a petition for a
Conditional Use permit for the property located at 618 E. Golf Road ("Property") and legally described as
follows:
THAT PART OF THE WEST 325.71 FEET (AS MEASURED AT RIGHT ANGLES TO THE WEST LINE
THEREOF) OF LOT 7 IN OWNERS SUBDIVISION OF SECTION 13, TOWNSHIP 41 NORTH RANGE 11

Property Index Number(s) 08-13-200-025-0000, 08-13-200-026-000 and 08-13-200-027-0000; and
WHEREAS, the Petitioner seeks a Conditional Use permit to operate an adult daycare center for the property
located at 618 E. Golf Road; and
WHEREAS, a Public Hearing was held on the request for a Conditional Use permit being the subject of PZ -
24 -18 before the Planning and Zoning Commission of the Village of Mount Prospect on the 24th day of
January, 2019, pursuant to proper legal notice having been published in the Daily Herald Newspaper on the
9th day of January, 2019; and
WHEREAS, the Planning and Zoning Commission has submitted its findings and recommendations to the
President and Board of Trustees in support of the request being the subject of PZ -24-18; and
WHEREAS, the Mayor and Board of Trustees of the Village of Mount Prospect have given consideration to
the request herein and have determined that the same meets the standards of the Village and that the
granting of the proposed Conditional Use permit would be in the best interest of the Village.
NOW, THEREFORE, BE IT ORDAINED BY THE MAYOR AND BOARD OF TRUSTEES OF THE VILLAGE
OF MOUNT PROSPECT, COOK COUNTY, ILLINOIS:
SECTION ONE: The recitals set forth are incorporated as findings of fact by the Mayor and Board of
Trustees of the Village of Mount Prospect.

SECTION TWO: The Mayor and Board of Trustees of the Village of Mount Prospect do hereby grant a
Conditional Use permit for an adult daycare center for the property located at 618 E. Golf Road subject to the
following conditions:
1. "Provide architectural plans which include:
a. A building code analysis reflecting existing and proposed conditions;
b. All egress, uses, types of construction, occupant load, and any necessary changes due to the
change in use group per the International Building Code (IBC);
2. An inspection from the Illinois Department of Aging shall be performed prior to granting occupancy; and
3. Compliance with all Development, Fire, Building, and other Village codes and regulations."
SECTION THREE: The Village Clerk is hereby authorized and directed to record a certified copy of this
Ordinance with the Recorder of Deeds of Cook County.
SECTION FOUR: This Ordinance shall be in full force and effect from and after its passage, approval and
publication in the manner provided by law.
