In the current budget, funds have been requested to continue combined and separate sewer syste
repairs utilizing the cured -in -place -pipe (CIPP) process. This process entails rehabilitating existing sewe
main segments by inserting a cured -in-place (CIPP) liner inside a host pipe segment. This method is mor
cost effective than open -cut excavation and far less disruptive. The Village has successfully utilized thi
process since the late 1970s. I
The combined sewer repairs were identified during the review of the Combined Sewer System Evaluation
Study (CSSES) completed in 2015.
The separate sanitary sewer repairs were identified during our in house televising program that is part of
the revised Inflow and Infiltration Control Program (IICP) of the Metropolitan Water Reclamation District of
Greater Chicago (MWRDGC).
A two (2) - year contract to install CIPP was awarded to Hoerr Construction of Goodfield, Illinois at the
April 3, 2018 Village Board meeting. 2019 is the second, and final year of work authorized in this contract.
To effectively monitor Hoerr's work quality and assure end -product quality, specific technical skills and
experience are required. Specifically, technical monitoring of the pipe cleaning process, liner inversion,

curing, material core laboratory analysis, and post -construction video review are some of tasks required to
successfully prosecute the project. Staff prepared a detailed request for proposals (RFP) necessary to
solicit these services. A copy of this RFP document is attached.
Request for Proposals (RFP)
Consulting engineers were asked to submit a technical proposal detailing the firm"s background, key
personnel, and approach to the project. In addition, each firm was also asked to submit a separate cost
proposal indicating work effort anticipated and a not -to -exceed fee. The required scope of work includes
30,000 linear feet of pipe ranging in diameter of 8" to 18" and an estimated installation cost of $1.3
million. The locations of proposed pipe segments included in this year's project scope are depicted on the
attached map.
RFPs were distributed to three (3) engineering firms with expertise in the in construction rehabilitation o
sewer main through means of cured -in-place pipe lining. I
Two (2) technical proposals and two (2) cost proposals were received out of three (3) firms staff reached
out to. Each proposal was evaluated based on the background of the firm, related project experience,
proposed services and scope of work, schedule work effort and proposal content. Each firm received a
point ranking (total points = 100) for each criterion. The table below summarized this analysis:

The highest rated firm was Baxter & Woodman of Crystal Lake, IL. Baxter & Woodman has performed thi
type of work for the Village for the past three years. They have performed exceptionally well and
understand the expectations of the Village for a project of so much importance. Their staff has personnel
who are certified in NASSCO PACP coding and have experience entering data into the Village"s assessmen
database. All references checked indicated that Baxter & Woodman performed well. 11
In addition, Baxter & Woodman has successfully completed and are currently working on several
engineering projects for the Village over the course of the last 10 years. These projects include sanitary
flow monitoring, sanitary and combined sewer spot repair projects, CCTV observation and coding, as well
as design and construction observation services for our 2018 water main replacement project.
Each firm was asked to submit a cost proposal as well. The table below summarizes staff's analysis of
costs.

The lowest cost proposal was submitted by Baxter & Woodman. A copy of Baxter & Woodman"s submitted
cost proposal is enclosed. It is the opinion of staff that Baxter & Woodman has allocated sufficient work
effort (645 hours) to successfully prosecute this project.
Staff recommends including a 10% contingency in the award for this proposed contract because time
spent on construction inspection can vary due to circumstances beyond the engineering consultant"s
control. For example, contractor delays, material availability and inclement weather are all parameters
that can lengthen anticipated construction time.

It is the staff opinion that Baxter & Woodman fully understands the proposed project, has prepared a
thorough proposal based on extensive experience, is well qualified to perform the work and has not
increased their rates since 2016.

Summary
The Village is requesting a Proposal to observe and inspect sewers to be lined as part
of the 2019 CIPP program. The Village will be lining 25,900 linear feet of both combined
and separate sanitary sewer mains. The pipe sizes vary from 8" to 18" with over 750
service reinstatements. The list was put together from past CCTV efforts done by both
Village staff and a contractor. All pipe segments were graded to NASCCO standards. All
pipes being lined are graded a 4 (see attachment A).
Problem Statement
The Village seeks a qualified Consultant to provide specified engineering services necessary to
let, and implement a CIPP Lining project utilizing contracted resources. The scope and scale of
this proposed project will be limited by available funding.
Project Scope
The consultant will provide professional engineering services and provide, at a minimum, the
following:
Construction Management and Inspection Phase
1. Consultant shall conduct a preconstruction conference with representatives of the
Owner, Contractor(s), Utilities and other interested parties affected by the
proposed construction.
2. Consultant shall provide resident engineers, inspectors, technicians, and
surveyors for the project as follows:
Provide a Resident Engineer/Inspector (RE/1) for full time daily
observation of the work during the construction period to observe
and record construction progress under the general guidance of
the Engineer. On the basis of on-site observation, the Engineer
will keep the Owner informed of the progress of the work, shall
endeavor to guard the Owner against defects and deficiencies in
the work of the Contractors, and may disapprove or reject work
as failing to conform to the contract documents.
Prior to the start of construction, the Consultant shall make a
digital photographic survey of the targeted work zone(s) to
document existing conditions of both public and private property?
The survey shall be conducted at a scale sufficient to note defects
in driveways, sidewalks, structures, and landscaping in or
adjacent to probable construction work areas.
iii. Prepare written daily inspection reports. Reports are to be
delivered to Owner in an electronic format. Digital photographs
documenting work tasks in progress as well as any potential
problems or unusual conditions shall also be included with each
report.
iv. Review Post lining footage to ensure all service laterals have
been reinstated.
V. Review post lining footage videos and enter date into the Villages
asset management database.
vi. Assist the Owner in preparation of any necessary change order
and/or field directives.
vii. Process monthly applications for payment submitted by the
Contractor(s), and forward to Owner with recommendations for
approval and payment.
viii. Assist the Owner with the resolution of inquiries, complaints, or
concerns raised by residents or the general public.
ix. Review shop drawings and other equipment/material submittals
as may be required by the contract documents.
X. Perform a field observation of all completed work before a final
application for payment is processed for the Contractor.
A. Revise the original plans in accordance with construction
observation records of the Resident Engineer, Inspectors, and the
Contractor, and supply the Owner with one set of as -built
drawings in a suitable electronic format.

Technical Proposal
The Consultant's technical proposal shall be organized so that the outline generally follows the
format of this request for proposals. The proposal shall cover the approach and the methodology
the Consultant intends to use in carrying out the project. The technical proposal should also
include statements setting forth the Consultant's line of business, organizational structure,
summary of past experience, and specific descriptions of at least three (3) projects completed in
the Chicago metropolitan area that are similar in subject, size, and complexity to the scope of
services specified herein. These descriptions must include contact information for reputable
references that can attest the Consultant's representations.
Cost Proposal
The consultant will quote a not -to -exceed price for each phase of the services described under
Scope of Services. Each price quote should include a breakdown by personnel classification and
estimated number of hours spent by each. Price proposals can be separated into minimum and
recommended hours for field observation. Proposals shall also include a price for additional
segments added to the project. Price proposals should be submitted as a distinct document in a
separate sealed envelope marked:

Present Work Load
A complete tabulation of the Consultant's current project workload is to be included. Each project
shall be identified by name, type, size, duration, and personnel assigned.
Personnel
Professional resumes, including education, work experience, professional credentials, and
employment status (e.g. full-time, part-time, years with the firm, job description) for each individual
to be assigned to the project described herein should be provided.
Disqualification of Proposals
More than one proposal for the same work described herein from an individual, firm or partnership,
a corporation or an association under the same or different names, will not be considered.
Reasonable grounds for believing that any party is interested in more than one proposal for the
work contemplated will cause the rejection of all proposals in which said party is interested. If
there are reasonable grounds for believing that collusion exists among the parties submitting
proposals, the proposals of the participants in such collusion will not be considered.
Licenses
Each party submitting a proposal shall possess all necessary federal, state, and local licenses as
are required by law, and shall furnish satisfactory proof to the Village upon request that the
licenses are in effect during the entire period of the Contract.
Award of Contract
The award of any Contract may be made to the Consultant who submits the proposal which is in
the best interest of the Village as evaluated by Village staff. Such evaluations will include the
experience of the Consultant, capability of the Consultant, and other factors deemed by Village
staff to be in the best interest of the Village.
In addition, the Village reserves the rights to accept or reject any and all proposals, to waive any
informality in any proposals, and to accept any proposals considered advantageous to the Village.

Baxter & Woodman has been assisting the Village with
construction observation for your sewer system improvements
since 2005. Improvements have included CIPP Lining and Spot
Relays that average an estimated $1 million annual construction
value. Our tasks have included:

VILLAGE OF MOUNT PROSPECT
Combined Sewer System Evaluation, GIS, Design,
and Construction Assistance (ongoing)
Baxter & Woodman has been providing the Village of Mount
Prospect with sewer evaluation studies, GIS services, and
design and construction services for the combined sewer
rehabilitation program since the early 2000s.
The Village's sewer system consisted of a separate
wastewater and storm sewer system as well as a
combined sewer system that handled both wastewater
and stormwater run-off. In December 2002, the Village
Board contracted Baxter & Woodman, Inc. to conduct a
comprehensive assessment of the Village's combined sewer
system. The scope of the project included an inventory and
mapping of sewer mains, manholes, inlets, catch basins,
and other structures in the combined sewer area. In
total, over 260,000 linear feet of Village -owned combined
sewers were televised and evaluated. Each sewer was
given a condition score and a multi-year rehabilitation
program was developed based on sewer ranking and the
Village's annual financial capabilities.
In 2015, the Village commissioned another program to
re -televise the sewers that had not been rehabilitated to
determine their condition. This latest evaluation showed
continued deterioration of the sewers. Approximately
32% of the sewers fell into the Grade 4 and 5 categories,
and another 22% in the Grade 3 category (Grade 5 sewers
require rehabilitation within one to two years, Grade
4 within three to four years, and Grade 3 within five to
ten years). The majority of the structural defects can be
rehabilitated by lining the sewers with cured -in-place
pipe (CIPP). In most cases, CIPP can be installed without
excavation, although a few combined sewers will require
excavation and replacement of a section of pipe prior to
CIPP installation.

Approximately 18,000 feet of previously lined sewers was also televised to confirm the reliability
of the rehabilitation technology. The review showed that previous CIPP repairs remain intact and
in good condition. This is to be expected since CIPP repairs are designed for a minimum 50 -year
service life.

Sanitary Sewer Rehabilitation
Baxter & Woodman performed design engineering and
on-site inspection services for the 2014 critical sanitary
sewer repairs and 2015/16 sanitary sewer rehabilitation
projects. Eric Murauskas designed and observed the
installation of approximately 150,000 feet of Cured in
Place Pipe (CIPP), ranging in size from 8 -inch to 36 -inch
in diameter.

Eric is well versed in many areas including sanitary sewer rehabilitation, sanitary
sewer evaluation studies, sanitary sewer flow monitoring, stormwater drainage
studies, sewer system design, and municipal services. He also has developed strong
expertise in water supply well design, test well drilling programs, and water
treatment plant design.
Eric recently completed the CIPP Inspector Training and Certification Program
from NASSCO, Inc. NASSCO's Inspector Training and Certification Program (ITCP)
is a new standard national training and certification program that provides field
construction professionals with comprehensive learning and tools to understand
and inspect trenchless pipeline renewal technology.

Annual CIPP Lining Program Construction Inspection
Project Manager assisting the Village with construction observation for sewer
system improvements since 2005. Improvements have included CIPP lining and
Spot Relays that average an estimated $1 million annual construction value.
Hoffman Estates, IL
2013 Critical Sewer Repairs
2015-2016 Sanitary Sewer Rehabilitation
Project Manager for the design and construction engineering for the rehabilitation
of the sanitary sewer system using cured -in-place pipe (CIPP) for approximately
150,000 feet of cured in place pipe. The construction included placement of
approximately 710 feet of 18 -inch and 330 feet of 36 -inch diameter manhole -to -
manhole sanitary sewer lining; and rehabilitation of five manholes to repair and
protect the structures from hazardous chemicals corrosion.

Sanitary Sewer Rehabilitation
Project Manager for the design engineering and on-site inspection services for the
sanitary sewer rehabilitation project. Eric Murauskas designed and observed the
installation of approximately 15,000 feet of Cured in Place Pipe (CIPP), ranging in
size from 8 -inch to 42 -inch in diameter.

2013 Storm Sewer Repairs
Project Manager for a condition assessment of approximately 5,500 feet of storm
sewer and development of a prioritized rehabilitation program. The condition
assessment of the sewer pipes was made from review of television inspection
videos. The rehabilitation design was developed using a combination of cured -in-
place pipe (CIPP) and open excavation methods. The construction included
placement of approximately 1,700 feet of manhole -to -manhole storm sewer lining,
ranging in size from 10 -inch diameter to 21 -inch diameter; and open cut spot
repairs in four locations.
