Mayor Juracek stated this public hearing and public meeting, called pursuant to proper legal notice
having been published in the Daily Herald on September 9, 2016, is to consider several annexations
and zoning designation of [11 (Limited Industrial)] territory within unincorporated Cook County to the
Village of Mount Prospect.
Mayor Juracek called the Public Hearing to order at 7:50 pm to consider the zoning designation of 11
(Limited Industrial) for the territory within unincorporated Cook County being considered for
annexation into the Village of Mount Prospect. Mayor Juracek explained the Public Hearing is for the
zoning designation but the ordinance to be considered is for both the annexation and zoning
designation.
Community Development Director William Cooney provided background information regarding the
properties located at the three proposed annexations. The uses in the proposed annexed areas are
mostly industrial. Staff recommendation is to bring in these properties with 1-1 Limited Industrial
zoning designation.
Staff invited affected property owners to meet and discuss the impacts of the proposed annexation.
The meeting was held on Tuesday, September 13tH
There were no questions regarding the proposed 1-1 Limited Industrial zoning designation for the
subject properties.
Mayor Juracek closed the Public Meeting at 7:53 pm.
Village Manager Michael Cassady provided additional background information regarding the
properties proposed to be annexed.
■ Algonquin Road is owned by IDOT and they will continue maintain and plow the road.
■ Some property owners that attended the open house expressed concerns regarding flood
control, drainage and lighting.
■ Nordic Road is in good condition.
■ The Village is coordinating with the township to develop a strategy to improve Malmo Drive.
Fire Chief Brian Lambel provided background information regarding the Elk Grove Township Fire
District (EGTFD) fire and emergency medical services and provided the following in response to
questions from the Village Board:
• Elk Grove Township Fire District (EGTFD) contracts with a private firm for fire and emergency
medical services. — 15 full-time firefighters, 15 paid on call, and a few administrative staff.
• In 2017 EGTFD should be able to provide services to properties outside of the annexations
and staff is looking at a long term plan and analyzing the financial and operational needs of
EGTFD and the Village.
• Meetings with EGTFD started about 8 months ago.
Village Attorney Lance Malina provided the following in response to questions from the Village Board:
• Possible intergovernmental agreement between the Village, EGTFD and surrounding
communities to ensure services are provided to incorporated and unincorporated areas. This
is on the rational basis since EGTFD will lose EAV through the annexations.
• Under state statute park districts has an opt -out from annexations and the park district board
would need to consent to the annexation.

• Statutory payments to EGTFD (five years) to account for loss of EAV
There was general discussion regarding Cook County's request for municipalities to annex
unincorporated areas and Elk Grove Township Road District

• Proposed postponing the annexations before the village board and suggested the Village and

Mayor Juracek stated Item B has been withdrawn, the petitioner was requesting a conditional use that
was necessitated by her need to move the business and she has since renegotiated a lease
extension at her current location.

Mayor Juracek presented for a first (1S) reading An Ordinance Amending the Official Zoning Map of
the Village of Mount Prospect for Property Located at 1000 South Busse Road, 1020 Addison Court,
1025 Addison Court, 1030 Addison Court, 1034 Addison Court, 2201 South Busse Road.
Community Development Director William Cooney provided background information.
These properties were located in unincorporated Cook County prior to its annexation into Mount
Prospect in June 2016. Upon annexation, the Subject Property was automatically classified R -X
Single -Family Resident District. The Village is pursuing the rezoning from R -X Single Family
Residence to 1-1 Limited Industrial. These properties contain several commercial and industrial
buildings and the Village Comprehensive Plan designates these properties as Light Industrial and
Office Research and General Industrial.
The Planning & Zoning Commission held a public hearing on September 8th and recommended
approval by a vote of 6-0.

Increment Financing Interested Parties Registry and Adopting Rules for Registration in Relation
Thereto.
Mr. Cooney provided background information and responded to questions from the Village Board.
Pursuant to the provisions of state statue the Village is required to establish a Tax Increment
Financing Interested Parties Registry and adopt registration rules for said TIF Interested Parties
Registry, relative to the proposed establishment of the existing Downtown TIF District and the
proposed Prospect and Main TIF District.

With a quorum of members present, Mayor Pro Tem Saccotelli announced that Trustee Matuszak
requested to participate in tonight's meeting via telephone conference. Trustee Matuszak cited an out-of-
town business trip as the reason for this request. Mayor Pro Tem Saccotelli asked for a motion to accept
Trustee Matuszak's request to participate in the meeting via telephone.
Trustee Hoefert, seconded by Trustee Zadel moved to accept the motion.
By a unanimous vote, Trustee Matuszak was approved for participation by telephone.

Northwest Suburban Alliance on Domestic Violence. Ms. Bran thanked the Mayor and Board for their
support of this important issue.

Communications Director Howard Kleinstein presented awards recently won at the
"Best of the Midwest Media Fest Awards", a contest run by Wisconsin Community Media and Midwest
Region of the Alliance for Community Media. Mr. Kleinstein stated MPTV won awards in all categories
entered.

Village Attorney Lance Malina responded that this proposed amendment deals with infrastructure. The
amendment was designed to prohibit the Illinois Legislature from using transportation funds for non -
transportation related projects. Specifically, legislators would be permitted to spend revenue derived from
taxes, fees, or licenses on vehicles, fuels, transportation infrastructure, and transportation operations on
transportation costs and projects.

A. A resolution in support of Raptor 4, LLC, 1401 Feehanville Drive, Mount Prospect, Illinois, making
application for a Cook County Class 6b Tax Abatement.
Community Development Director Bill Cooney presented this resolution that would grant approval
of a Class 6b property tax incentive for Raptor 4 LLC (Raptor) at 1401 Feehanville Drive in the
Kensington Business Center. Raptor has entered into a contract to purchase the 30,000 square
foot building, subject to approval of the 6b incentive. If approved, Raptor would construct a
30,000 square foot addition to the building and ultimately sell the property to Parenti-Raffealli who
would relocate from their downtown location into the new facility.

(Prospect and Main Tax Increment Financing District)
Village Manager Michael Cassady stated this is a procedural action under the guidelines of the
TIF Act, to give notice and announce the public hearing and a Joint Review Board(JRB) meeting
on November 7, 2016 at 3:00pm.The JBR meeting is an opportunity to meet with colleagues from
local government to present the TIF Eligibility Study and Redevelopment Plan and Project. It will
include our schedule and vision for the new TIF district and the termination of the existing TIF. In
addition to this meeting, Mr. Cassady invited residents to attend the Prospect and Main TIF Open
House on November 9th at 4:00 in the Village Board room followed by the Committee of the
Whole meeting at 7:00pm. These are both educational opportunities providing interested parties

The total estimate of cost for engineering and construction is $325,400. The federal share
would be $232,320 and the village share would be $93,080.
Projected completion of project — by the end of 2017
Rand Road Street Lighting Improvements
o Construction of streetlights along Rand Road from Henry Street to Main Street (Route 83)
o The total estimate of cost for engineering and construction is $ 1,071,840. The federal share
would be $840,840 and the village share would be $231,000.

Public Works Director Sean Dorsey provided background information including bid process and
results, staff recommendations and responded to questions from the Village Board.
• The skeleton of the existing system is approximately 15 years old, comprising of a digital base
station, 15 desktop radio consoles, 120 mobile radios, and 5 handheld radios
• Staff proposes an upgrade to a digital half -duplex repeater system which will improve
communication to those areas in the village where communication is weak
• Despite actions to attract multiple bidders, only one, Alpha Prime Communications of Northbrook,
was received. The bid was checked to ensure that the supplier was providing the necessary
equipment and satisfied specifications.
• Staff reviewed Alpha Prime Communication's references and found them to be reliable and
acceptable

"WORLD PANCREATIC CANCER DAY"
November 17, 2016
WHEREAS, November 17th is declared as "World Pancreatic Cancer Day" in the Village
of Mount Prospect; and
WHEREAS, in 2016, an estimated 53,070 people will be diagnosed with Pancreatic
Cancer in the United States and 41,780 will die from the disease; and
WHEREAS, Pancreatic Cancer surpassed breast cancer this year to become the third
leading cause of cancer deaths in the United States and is projected to become the
second by 2020; and
WHEREAS, Pancreatic Cancer is the only major cancer with a five-year relative survival
rate in the single digits at just 8 percent; and
WHEREAS, when symptoms of Pancreatic Cancer present themselves, it is generally in
later stages, and 71 percent of Pancreatic Cancer patients die within the first year of
their diagnosis; and
WHEREAS, approximately 1640 deaths will occur in Illinois in 2016; and
WHEREAS, Pancreatic Cancer is the 7th most common cause of cancer -related death in
men and women across the world; and
WHEREAS, there will be an estimated 418,451 new Pancreatic Cancer cases diagnosed
globally in 2016; and
AND WHEREAS, the good health and well-being of the residents of Mount Prospect are
enhanced as a direct result of increased awareness about Pancreatic Cancer and
research into early detection, causes, and effective treatments.
NOW, THEREFORE, I, Arlene A. Juracek, Mayor of the Village of Mount Prospect, do

Honoring the Life and Legacy of
Lisa Angell
Whereas, Lisa Angell served the Village of Mount Prospect faithfully and with
great distinction for 28 years beginning her career as a member of the Mount
Prospect Public Works Department, promoted to Solid Waste/Environmental
Services Coordinator in 1991 and was ultimately appointed Village Clerk on July
5, 2005; and
Whereas, Lisa Angell proudly hailed from the state of Iowa where she earned
her Bachelor of Arts in Social Work and Master of Arts in Public Administration
from the University of Iowa and never missed an opportunity to connect with a
fellow Hawkeye, and
Whereas, Lisa Angell was bestowed the Shining Star at the Celestial
Celebration on February 7, 1998 for her role in the Village's Solid Waste
Program, "Waste Not, Want Not.'; and
Whereas, Lisa Angell served as staff liaison on the Solid Waste Commission
and while serving as the Public Works Solid Waste Coordinator, she helped
develop the Villages'recycling program and she would often refer to herself as
the Villages' official "dumpster diver'; and
Whereas, Lisa Angell personified the motto of the Village she so dearly loved,
"Friendliness is a Way of Life"; greeting residents daily with her warm smile and
kind words, and
Whereas, as Village Clerk, Lisa Angell wore many hats, serving as a Freedom
of Information Officer, Deputy Registrar, the Official Keeper of Village Records
and her favorite title, `general information';
Whereas, Lisa Angell volunteered her time to "Meals on Wheels" and the
Backpack Ministry at St. Raymond's Parish in Mount Prospect, donated blood
regularly at the Village's monthly blood drive and was a mentor with the Human
Services Mentor Program, and
Whereas, Lisa Angell proudly served her fellow clerks as Vice President of
Programs for the Municipal Clerk's Association of the North and Northwest
Suburbs, and
Whereas, as a highly regarded and respected leader, Lisa Angell brought pride,
integrity and professionalism to the workplace every day; and
Whereas, Lisa Angell, an advocate for health and wellness, walked to work
every day in rain, snow, or sun, and
Whereas, as Village Clerk, administered the Oath of Office to public servants
who considered it an honor to receive their official appointment from Lisa Angell;
and
Whereas, Lisa Angell, the quintessential professional, never allowed her illness
to interfere with her duties as Village Clerk, who came to work each day with a
cheerful smile and bright optimism making everyone around her forget the battle
she quietly and bravely was fighting, and

Whereas, Lisa Angell will be remembered as a devoted mother whose children
have had a positive and productive presence in the Village, a valued employee
who, even as her illness progressed, rarely missed an entire day of work in 28

fill, trusted co-worker to the many who considered it a privilege to have worked
beside her, a proud and respected member of the Mount Prospect community
who presence will be sorely missed, and
Whereas, Lisa Angell passed into eternal life on October 17, 2016 just as she
lived every day of her life, with beauty, grace and dignity.
NOW THEREFORE, BE IT RESOLVED BY THE MAYOR AND BOARD OF

Information:
On Monday, October 24, 2016 the Village Manager and Administrative Assistant to
the Village Manager met with applicant Yoshi Nara, currently the General Manager of
Ramen Misoya located in Mount Prospect, who is seeking to obtain a class 7-2"
liquor license. The class "F-2" license provides for the sale of beer and wine at
dining tables.
Misoya Chicago is a Japanese based corporation and in 2011 expanded to become
part of a global chain of ramen restaurants. The Mount Prospect location opened
its doors in 2013 and is the fourth location in the Continental U.S. The success of
this location prompts the request for a liquor license.
The total building square footage is 2,200 square feet with an estimated seating for
forty. Ramen Misoya's dining room is simple but handsome, packed with wooden

ramen served in a bowl of broth and garnished with various combinations of protein
and vegetables. The restaurant is opened seven days a week from 11:30 a.m.-10:00
p.m.
Discussed with the applicant were the requirements of the Village's Liquor Code
including the need to be mindful of sales to minors as well as obviously intoxicated
persons. The requirements of the Village's Fight Ordinance were also discussed.
The Village Manager also emphasized the need for appropriate controls to be in
place to insure that restaurant employees do not inadvertently violate code
requirements. Mr. Nara did complete Basset classroom training and indicated he
understood his obligations in this regard.

(ALCOHOLIC LIQUORS) OF THE VILLAGE CODE OF MOUNT
PROSPECT
Department: Village Manager's Office
Information:
On Wednesday, October 26, 2016, the Village Manager and Administrative Assistant
met with owner Bill Wilson and general manager Giovanni Labbate regarding his
request for the creation of a new Class "F -V liquor license. Bricks Wood Fired Cafe
will be the featured restaurant in the newly constructed Mount Prospect Ice Arena
located in the Kensington Business Center. Class "F -V license provides for full
service of alcoholic beverages at dining tables only.
Mr. Wilson has extensive knowledge in the hospitality pizza industry and Mount
Prospect will be his fifth Bricks location all of which operate with a liquor license.
This fast casual restaurant is 3,000 square feet with an estimated seating for ninety.
He indicated that the uniqueness of the brick oven cooking process is what
distinguishes Bricks pizza from others. The thin -crust, Neapolitan -style pizza will be
the centerpiece of the menu. The restaurant will be open seven days a week
Monday through Thursday from 11:00 a.m. to 9:00 p.m., Friday and Saturday until
10:00 p.m. and noon until 8:00 p.m. on Sunday.
Among the items discussed with the applicant were the requirements of the Village's
Liquor Code including the need to be mindful of sales to minors as well as obviously
intoxicated persons. Also discussed was the need for new managers to be
fingerprinted and all employees involved in the delivery of alcohol to complete a
certified liquor servers training course. The requirements of the Village's Fight
Ordinance were reviewed with an emphasis regarding the need for appropriate
controls to be in place to ensure that restaurant employees do not inadvertently
violate code requirements.

Item XI. A.: MOTION TO ACCEPT SUBURBAN PURCHASING COOPERATIVE
BID RESULTS FOR THE PURCHASE OF THREE (3) NEW
PICKUP TRUCKS IN AN AMOUNT NOT TO EXCEED $85,258
Department: Public Works Department
Information:
In the current budget, there are funds available to replace pickups 2710, 2715, and
4500. These trucks are all 2004 GMC Sierra 2 -wheel drive, 3/4 -ton pickups with
service bodies used by the Public Works Department for transportation, hauling
supplies/tools and towing trailers/equipment to jobsites throughout the Village.
These vehicles have been evaluated utilizing our detailed comprehensive
replacement policy. This policy uses a point -based criterion, which rates four (4)
major factors, when considering a unit for replacement: Age, mileage, repair &
maintenance (R&M) cost, and condition of the body/mechanical components. A
point total equaling or exceeding eleven (11) out of sixteen (16) indicates that the
vehicle should be recommended for replacement.
The vehicles scored in a range between thirteen (13) and fifteen (15) points
indicating a very high rating and should be placed on the highest priority to be
replaced. The vehicle condition evaluation forms and summary reports for each
vehicle have been attached as Exhibits A and B respectively. Additionally, included
is the life cycle cost analysis for each vehicle as Exhibit C, which breaks out our total
ownership cost for each vehicle. A photograph of each existing vehicle has been
included as Exhibit D.
Recently, Mercury Associates, Inc. performed a study on Optimal Vehicle
Replacement Cycle Analyses for several of our fleet asset classes, including 2 -
wheel drive pickup trucks. The result of their analysis indicated the optimal
replacement cycle for our 2 -wheel drive pickup trucks is at year ten (10) and further
recommended replacing after eight (8) years based on vehicle complexity and
efficiency.
If the service life is extended, each vehicle will require repairs to be performed. The

summary cost for each vehicle is listed below. Detailed listing of necessary repairs
can be found in Exhibit A and B.

The proposed replacement vehicle is a Ford F-250 Super Cab. The F-250 Super
Cab has a slightly larger cab to accommodate the storage of some equipment
inside the cab to protect the equipment from weather. All three (3) vehicles currently
utilize a fiberglass service body. Staff evaluated the existing service bodies and has
determined the service bodies to be in good condition. Staff will remount the
existing service bodies on the new pickups. This will save an aggregate amount of
approximately $30,000 compared to purchasing new fiberglass service bodies.
Staff proposes to procure the three (3) replacement vehicles through the provisions
of Suburban Purchasing Cooperative (SPC) joint bid. The SPC contract is publicly
bid and satisfies the public bidding requirements of the Village of Mount Prospect
Purchasing Policy.
Funds for this proposed purchase are currently 100% funded through the Vehicle
Replacement Fund and are available in the 2016 budget.
Delivery of the new vehicles is expected to be within ninety (90) days of the
purchase order. The new vehicles will be inspected at the dealer in Frankfort, Illinois
before being driven to our location for acceptance. The vehicles being replaced will
be sold through the GovDeals.com internet auction.
Alternatives:
1. Accept Suburban Purchasing Cooperative joint bid results for the purchase of
three (3) 2 -wheel drive pickup trucks.
2. Discretion of the village board.
Budget Impact:
The proposed award will require the expenditure of $85,258 from the Vehicle
Replacement Fund. Sufficient funds exist in the current budget.
Recommendation:
Staff recommends that the village board accept the Suburban Purchasing
Cooperative (SPC) bid results and authorize the purchase of three (3) new 2017
Ford F-250 Super Cab/Chassis from Currie Motors, of Frankfort, Illinois at a cost

These trucks are 2004 GMC Sierra, 3/4 -ton pickups. Currently, 4526 is 2 -wheel drive
with a service body and 4540 is 4 -wheel drive with a snowplow. They are used by
the Public Works Department for transportation, hauling supplies/tools, snowplowing
and towing trailers/equipment to jobsites throughout the Village.
These vehicles have been evaluated utilizing our detailed comprehensive

Item XI. C.: MOTION TO ACCEPT SUBURBAN PURCHASING COOPERATIVE
BID RESULTS FOR THE PURCHASE OF ONE (1) FULL SIZE
CARGO VAN IN AN AMOUNT NOT TO EXCEED $27,500

Information:
In the current budget, there are funds available to replace pickup truck 4529. This
truck is a 2004 GMC Sierra 2 -wheel drive, % ton pickup with a service body used by
the Public Works Department for hauling supplies/tools and towing
trailers/equipment to jobsites throughout the Village. The Streets/Buildings Division,
in their daily maintenance of buildings, uses 4529 to haul materials and equipment.
This vehicle has been evaluated utilizing our detailed comprehensive replacement
policy. This policy uses a point -based criterion, which rates four (4) major factors,
when considering a unit for replacement: Age, mileage, repair & maintenance
(R&M) cost, and condition of the body/mechanical components. A point total
equaling or exceeding eleven (11) out of sixteen (16) indicates that the vehicle
should be recommended for replacement.
This vehicle scored a total of fourteen (14) points indicating a very high rating and
should be placed on the highest priority to be replaced. The vehicle condition
evaluation form and summary report have been attached as Exhibits A and B
respectively. Additionally, a life cycle cost analysis has been included as Exhibit C,
which breaks out our total ownership cost. A photograph of 4529 is included as
Exhibit D.
Recently, Mercury Associates, Inc. performed a study on Optimal Vehicle
Replacement Cycle Analyses for several of our fleet asset classes, including 2 -
wheel drive pickup trucks. The result of their analysis indicated the optimal
replacement cycle for our 2 -wheel drive pickup trucks is at year ten (10) and further
recommended replacing after eight (8) years based on vehicle complexity and
efficiency.
If the service life is extended the following repairs should be performed:

Refurbish/paint rear body and rusted sheet metal $2,500
Replace ball joints, bushings and alternator $1,500
Replace headliner and rubber brake lines $900
Total Estimate of Cost: $6,900
The proposed replacement vehicle is a Ford Transit full size van. Staff has
evaluated the need of another pickup truck and instead is recommending a % ton full
sized cargo van to accommodate electrical and building supplies in compartments
and racks inside the vehicle. There is a operational need to increase the capacity of
supplies/tools stocked in the vehicle and allow for the dry transportation of building
supplies during poor weather. The increased storage capacity will reduce the
number of trips to purchase supplies resulting in increased efficiencies.
The purchase of the Ford Transit represents a $900 savings compared to replacing
the existing pickup with a new two (2) wheel drive pickup truck.
Staff proposes to procure this replacement vehicle through the provisions of
Suburban Purchasing Cooperative (SPC) joint bid. The SPC Contract is publicly bid
and satisfies the public bidding requirements of the Village of Mount Prospect
Purchasing Policy.
Funds for this proposed purchase are currently 100% funded through the Vehicle
Replacement Fund and are available in the 2016 Budget.
Delivery of the new vehicle is expected to be within ninety (90) days of the purchase
order. The new vehicle will be inspected at the dealer in Frankfort, Illinois before
being driven to our location for acceptance. The vehicle being replaced will be sold
through the GovDeals.com internet auction.
Alternatives:
1. Accept Suburban Purchasing Cooperative joint bid results for the purchase of
one (1) full size cargo van.
2. Discretion of the village board.
Budget Impact:
The proposed award will require the expenditure of $27,500 from the Vehicle
Replacement Fund. Sufficient funds exist in the current budget.

Staff recommends that the village board accept the Suburban Purchasing
Cooperative (SPC) bid results and authorize the purchase of one (1) new 2017 Ford
Transit Full Size Cargo Van from Currie Motors, of Frankfort, Illinois at a cost not to
exceed $27,500.
