RESOLUTION NO.
39-88
RESOLUTION APPOINTING DAVID C. JEPSON AS AUTHORIZED
AGENT FOR THE ILLINOIS MUNICIPAL RETIREMENT FUND
WHEREAS, the Illinois Municipal Retirement Fund (IMRF) requires each parti-
cipating unit of local government to appoint an Authorized Agent to act as an
agent of the governing body in IMRF matters; and
WHEREAS, David C. Jepson is an employee of the Village and qualified to be the
Authorized Agent of the VIllage to the Illinois Municipal Retirement Fund.
NOW THEREFORE, BE IT RESOLVED BY THE MAYOR AND BOARD OF TRUSTEES OF THE
VILLAGE OF MOUNT PROSPECT, COOK COUNTY, ILLINOIS:
SECTION ONE: That David C. Jepson is hereby appointed as the Authorized Agent
of the Village of Mount Prospect and its employees to the Illinois Municipal
Retirement Fund; and that as such he shall have the following powers and
duties:
A.
To determine and certify to IMRF which employees are covered by IMRF
and Social Security; to see that the proper IMRF member contributions
are withheld from employee's earnings; and to complete payroll.
reports and forward member and employer contributions to IMRF.
B.
To forward all communications directed to members and beneficiaries
by IMRF and to assist members and beneficiaries in filing applica-
tions for retirement annuities and other benefits.
C.
To keep the governing body informed regarding IMRF matters.
D.
To perform all administrative duties in connection with IMRF.
E.
To file a petition for nomination and to cast the ballot for election
of an Executive Trustee of IMRF.
F.
To have any and all powers which may and can be delegated to said
Authorized Agent by the Statutes of the State of Illinois.
SECTION TWO: Said appointment shall be effective and shall remain in force
until changed by subsequent resolution duly adopted. That David C. Jepson is
further authorized, as is the Village Clerk, to make all necessary communica-
tion to the Illinois Municipal Retirement Fund relative to this appointment
including the execution of IMRF Form No. 2.20 Notice of Appointment of
Authorized Agent.
SECTION THREE: This resolution shall be in force and effect from and after
its passage and approval according to law.
