Mount Prospect Firefighters Pension Fund Meeting
May 25, 2012
I. CALL TO ORDER — ROLL CALL
President John Malcolm called the meeting to order at 10:10 a.m.
Present upon roll call: John Malcolm — President
Joe Knoll — Secretary /Firefighter
Sam Baas — Retired Firefighter
Absent: Jeff Ricker — FOIA/OMA Officer /Firefighter
David Erb — Treasurer
Also present was Carry Collins the Pension Fund Attorney.
II. APPROVE AGENDA
Motion by Joe Knoll, seconded by Sam Baas to approve the Agenda as
submitted.
Motion was approved.
III. CITIZENS TO BE HEARD
None.
IV. NEW BUSINESS
A. Request for Pension Benefits.
Tom Willming has submitted an application for an Occupational Disease
Pension as of April 25, 2012.
Motion by Joe Knoll, seconded by Sam Baas to accept the application.
Motion was approved.
The Board discussed using the Fire Dept Doctor, Tom Willming's Doctor
and authorizing Attorney Collins to select a third Doctor to evaluate the
requirements for an Occupational Disease Disability Pension.
Motion by Joe Knoll, seconded by Sam Baas to approve the list of
Doctors.
Motion was approved.
Tom Williming has submitted a request for Retirement Benefits for 29
years, 15 days of service.
This request is for an interim period until his Occupational Disease
Pension request is finalized.
Motion by Joe Knoll, seconded by Sam Baas to accept the application.
Motion was approved.
X. ADJOURNMENT
Sam Baas, seconded by Joe Knoll moved to adjourn the meeting at 10:33
a.m.
Motion was approved.
