A RESOLUTION FOR THE HOLIDAYSEASON
WHEREAS, the holiday season has arrived, and it is the time of the year
when, as individuals and as a community, we reflect on our relationships with
others, the goodwill that is shared, and all that remains to be done to create a
peaceful and safe world for all; and
WHEREAS, such a solemn, but joyous, time should include exchanges of
concern, care, and love among all people.
NOW, THEREFORE, BE IT RESOLVED BY THE PRESIDENT AND
BOARD OF TRUSTEES OF THE VILLAGE OF MOUNT PROSPECT,
COOK COUNTY, ILLINOIS:
SECTION ONE The President and Board of Trzustees of the Village of
Mount Prospect do extend to the citizens ofMount Prospect our sincere wishes
for a joyous holiday season, and on behalf of our entire community, we extend
to everyone a resolution ofgood faith and kindness so that there may be peace
on this earth.
SECTION TWO This Resolution shall be in fill force and effect from and
after its passage and approval in the manner provided by law.

PROCLAMATION
"NATIONAL DRUNK AND DRUGGED DRIVING (3D)
PREVENTION MONTH"
DECEMBER 2011
WHEREAS, motor vehicle crashes killed 927 people in Illinois during 2010; and
WHEREAS, hundreds of those deaths involved a driver impaired by alcohol and /or
drugs; and
WHEREAS, the December holiday season is traditionally one of the most deadly
times of the year for alcohol- impaired driving; and
WHEREAS, for thousands of families across the state and nation, holidays are a
time to remember loved ones lost; and
WHEREAS, organizations across the state and the nation are joined with the Drive
Sober or Get Pulled Over and other campaigns that foster public awareness of the
dangers of impaired driving and anti - impaired driving law enforcement efforts;
and
WHEREAS, the Village of Mount Prospect is proud to partner with the Illinois
Department of Transportation's Division of Traffic Safety and advocacy groups in
that effort to make our roads and streets safer.
NOW, THEREFORE, I, Irvana K. Wilks, Mayor of the Village of Mount Prospect do
hereby proclaim December 2011 as
"Drunk and Drugged Driving (3D) Prevention Month"
in the Village of Mount Prospect and do hereby call upon all citizens, government
agencies, business leaders, hospitals and other health care providers, schools and
public and private institutions to promote awareness of the impaired driving
problem, to support programs and policies to reduce the incidence of impaired
driving and to promote safer and healthier behaviors regarding the use of alcohol
and other drugs, this December holiday season and throughout the year.

AN ORDINANCE AMENDING CHAPTER 13 OF THE
VILLAGE CODE OF MOUNT PROSPECT
BE IT ORDAINED BY THE PRESIDENT AND BOARD OF TRUSTEES OF THE VILLAGE OF MOUNT
PROSPECT, COOK COUNTY, ILLINOIS ACTING IN THE EXERCISE OF THEIR HOME RULE POWERS:
SECTION ONE: That in accordance with Subsection A of Section 13.108 of Chapter 13 of the Village Code of
Mount Prospect, as amended, the Corporate Authorities shall determine the number of Liquor Licenses
available in each Classification and shall establish a list specifying the number of Liquor Licenses authorized.
Said list is hereby amended by decreasing the number of "Class F -1" Liquor License by One (1) SHO
RESTAURANT INC. d /b /a SAKURA RESTAURANT and increasing the number of "Class S -6" Liquor
License by One (1) SHO RESTAURANT INC. d /b /a SAKURA RESTAURANT at 105 S. Main Street. Said
list is as follows:

SUBJECT: CRAVE PIZZA (FORMERLY PANZANO'S),
106 W. NORTHWEST HWY
REQUEST FOR CLASS "F1" LIQUOR LICENSE
On Tuesday, November 22, 2011, the Village Manager met with Mr. Rafael Manjarres Dickson
regarding his request for the creation of a new Class "F1" Liquor License for the above -
referenced restaurant. The location of the proposed restaurant is in Clocktower Plaza and was
most recently occupied by Panzano's Pasta Pizza and More. The Class "F1" license provides
for full service of alcoholic beverages at dining tables only. Mr. Dickson is the owner and sole
shareholder of the business.
While Mr. Dickson has no previous experience as a liquor license holder he formerly was a
supervisor of waiters at Kayla's restaurant in Chicago which served alcohol. Crave Pizza will be
a family oriented restaurant specializing in thin crust pizza. The menu will also include
appetizers, hamburgers and steak sandwiches. The restaurant will be open seven days a week
from 4:00 p.m. to 11:00 p.m. with 80% of the business being carry outs. There will be no change
to the interior of the restaurant and Mr. Dickson anticipates a December 9 opening.
The Village Manager discussed at length with the applicant the Village's particular sensitivity to
the sale of alcohol to minors and over service to clearly intoxicated individuals. The Village
Manager informed Mr. Dickson that he is responsible for the actions of all employees and that it
was imperative to establish an appropriate training program as well as management oversight
with regard to such sales. Mr. Janonis also covered the Village's Fight Ordinance requirements.
Mr. Dickson indicated that he had completed a certified liquor servers' course as a condition of
issuing the license.
A completed application along with all fees and paperwork are on file with the Village Manager's
office. Mr. Dickson has also submitted to fingerprinting and background checks. Should the
Village Board consider this application favorably the adoption of an Ordinance creating a new
Class "F1" liquor license should be adopted.
