A RESOLUTION AUTHORIZING A CONTRACT CHANGE
CONTRACT SUPPLEMENT NO.1
VILLAGE OF MOUNT PROSPECT. ILLINOIS
FOR - BITUMINOUS RESURFACING IMPROVEMENTS
HILLCREST It COUNTRY CLUB SUBDIVISIONS
MFT SECTION 11 CS
WHEREAS, It is deemed necessary and advisable for the best interest
of the Village of Mount Prospect that the contract between the said Village of
Mount Prospect and the Allied Asphalt Paving Company for the resurfacing
improvements in Hillcrest and Country Club subdivisions, MFT Section 11 CS
be changed by the construction of sidewalk aprons at various intersections,
by the removal and replacement of existing curb and gutter at the Southwest
corner of Willie and Gregory, and by the resurfacing of the adjacent existing
pavement at said southwest corner and
WHEREAS, The proposed change in the Contract will require the
addition of the following items of work:
1. Construct 2300 S. F. PCC Sidewalk (5")
2. Removal of 188 L.F. of combination curb and gutter
3. Construct 188 L. F. of combination cone curb and gutter special
4. Resurface 205 S. Y. Bit. Conc. Surface (I-II)
and,
WHEREAS, it is hereby determined that all data necessary to the'Oom-
plete analysis and to the establishment of costs of said changes hereinbefore
cited and referred to herein are made a part hereof, and
WHEREAS, The Letter Proposals 01 the Allied Asphalt Paving Company
copies of which are attached hereto and made a part hereof, agreeing to make
said changes hereinbefore cited and establishing the costs thereof be and the
same is hereby accepted for and in behalf of the Village of Mount Prospect,
Cook County, illinois
NOW. THEREFORE. BE IT RESOLVED, By the President and Board
of Trustees of the Village of Mount Prospect that the Contractor's Letter
Proposal and the establishment herein of the costs involved in making said
changes and the Contract Supplement attached hereto be accepted.
BE IT FUR THER RESOLVED, that the Allied A spbalt Paving Company
be authorized to perform the work set forth above; and that the Village Clerk
transmit to the Allied A sphalt Paving Company an executed copY' of the Con-
tract Supplement No. 1 and a certified copy of this Resolution

Dear Sirs:
We herewith agree to place additional concrete sidewalk, remove
and replace additional curb and gutter at various locations, and resurface
existing pavement on Willie Street just south of Gregory with bituminous
concrete surface course, Subblass I-II, as directed, all in connection with
our contract for Sec. 11 C. S. Mt. Prospect, Illinois., same to be paid for
at the contract unit prices for these items.
