FESOLUTION NO.

\V.HEREAS, the Village of Mount Prospect, located in the County of Cook,
State of Illinois, wishes to locate, construct, operate and maintain
watennains, sewer lines , driveways, alleys, street returns, sidewalks,
traffic signals and street lie'J'1ts constructed either by the Village of
Mount Prospect or under supervision of said Village in the year 1968
on the State Highways in the Village of Mount Prospect, Illinois
vnUCH by law comes under the jurisdiction and control of the Department
of Public Works and Buildings of the State of Illinois
and
WHEREAS, a permit from said Depart~nt is required before said work can
be legally undertaken by said Village or supervised by the Village;
NOW, THEREFDRE, BE IT RESOLVED BY THE VIILAGE OF MOUNT PROSPECT, COUNTY
OF COOK, STATE OF ILUNOIS:
SECTION ONE: That we do hereby request from the Depart~nt of Public
lvorks and BUildings, State of Illinois, a permit authorizing the Village
of Mount Prospect to proceed with the work herein described.
SECTION 'I\VO: That we hereby pledge the good faith of the Vi llage of
Mount Prospect and guarantee that all work shall be perfo:rm?d in accordance
with the conditions of the permit to be granted by the Department of Public
Works and Buildings of the State of Illinois, and to hold the State of
Illinois, Department of Public Horks and Buildings ha:rrnless on account of
any damages that may occur to persons or property during the prose.cution
of such work, and assume all liability for damages to persons or property
due to accidents or otherwise by reason of the work which is to be per-
formed under the provision of said permit.
SECTION THREE: That we hereby state that the proposed work is to be
performed by the errployees of the Village of Mount Prospect.
SECTION' FDUR: That the proper officers of the Village of T'10unt Prospect
are hereby instructed and authorized to sign said permit -n behalf of the
Village of Mount Prospect.
SECTION FIVE: This Resolution shall be in full force and effect from and
after its passage and approval in the manner provided by law.

THIRD: That we hereby state that the proposed work is,
is not, to be performed by the employees of the
Village ot Mount Prospect
FOURTH: That the proper officers of the Village ~t ~t Prospect
