Please take notice that on Monday, September 15,2008, at 1 :00 P.M., a public hearing
will be held before the Mount Prospect Police Pension Board at the Mt. Prospect Police
Department, -So S. Emerson, 3rd Floor Village Board Conference Room, Mt. Prospect,
Illinois, regarding the disability application ofthe above applicant, pursuant to the applicable
provisions of Article 3 of the Illinois Pension Code.
The public hearing will be held to receive evidence concerning the disability
application of the above applicant. The applicant is entitled to be represented by legal
counsel, to present any evidence and to r~spond to any evidence before the Board.
