A RESOLUTION OF INTENT TO PROVIDE A PLAN OF
OPERATION AND MAINTENANCE OF SEWAGE TREATMENT
WORKS
01 HEREAS, regulations of the U.S. Environmental Protection
gency for construction of sewage treatment works required
at the Village of Mount Prospect provide an assurance of
conomic, effective, and efficient operation and maintenance
f the sewage treatment works proposed under project
17 2238-02 in accordance with a plan of operation.
OW THEREFORE, BE IT RESOLVED BY THE MAYOR AND BOARD OF
I RUSTEES OF THE VILLAGE OF MOUNT PROSPECT, COOK COUNTY,
LLINOIS:
ECTION ONE: That prior to request for fifty percent (50%)
ayment for Step 3 grant a plan of operation will be prepared
I nd will include provision for:

1)
an operation and maintenance manual for each
facility
2)
3)
an emergency operating and response program
properly trained management, operation and
maintenance personnel
4)
5)
adequate budget for operation and maintenance
operational reports
6)
provisions for laboratory testing adequate to
determine influent and effluent characteristics
and removal efficiencies
SECTION TWO: That prior to request for fifty percent (50%)
payment for Step 3 grant, the village of Mount Prospect will
submit adequate evidence (for example, a draft of an operation
and maintenance manual) of timely development of an operation
and maintenance manual, and prior to request for ninety
percent (90%) payment of Step 3 grant, the Village of Mount
Prospect will furnish a satisfactory final operation and
maintenance manual.
SECTION THREE: That prior to request for fifty percent (50%)
payment for Step 3 grant,'the Village of Mount Prospect will
submit adequate evidence (for example, a draft of a sewer use
ordinance) of timely development of a sewer use ordinance, and
prior to request for eighty percent (80%) payment of Step 3
grant, the village of Mount Prospect will furnish a satisfactory
sewer use ordinance.

ECTION FOUR: That this Resolution shall be in full force
nd effect from and after its passage and approval in the
anner provided by law.
