June 25, 2015
CALL TO ORDER — ROLL CALL
President John Malcolm called the meeting to order at 9:04 a.m.
Present upon roll call: John Malcolm — President
David Erb —Treasurer
Joe Knoll — Secretary/Firefighter
Jim East — FOIA/OMA Officer/Firefighter
Sam Baas — Retired Firefighter
Also Present: Thomas Duda - Attorney
Cary Collins — Attorney
II. WRITTEN DECISION AND ORDER
Mr. Collins had completed the written decision and order, and submitted it
to the Board for approval.
After review, Mr. Erb wished to include the witness statement of injury and
the supervisor's report of injury as exhibits for the hearing.
Mr. Erb also wished to combine paragraphs 11 and 12 of the decision into
a single paragraph, and to append the incomplete paragraph 11 with the
term "applicant was referred to a pain management specialist".
Mr. East wished to correct the applicant's age inconsistency in paragraph
3 to read "forty-one (41)"
Mr. Knoll moved to grant duty -related disability benefits to Torrie Takoy's
request effective as of June 25, 2015. Motion was seconded by Sam
Baas.

Secretary Knoll — Yes
Mr. Baas —Yes
Mr. East —Yes
Motion Carried unanimously and the decision was circulated for
signatures.

III. ADJOURNMENT
Jim East moved to adjourn the meeting, seconded by Sam Baas.
Motion was unanimously approved at 9:11 a.m.
